Dr Casavantes has set up a patient assistance program for PMMA

A simple one page form for PMMA patient assistance is

available by writing to the program administrator at

pmmaassistance@....

The form is accepted via e-mail and can be filled out on a

computer and e-mailed by clicking a button or printed out and mailed in.

You will need a copy of Adobe Acrobat Reader to view and

fill it out. Acrobat Reader is a free program that can be downloaded at http://www.adobe.com/products/acrobat/readstep2.html.
