I hate to just forward this but I'm looking for some info and I wanted to make

sure I sent this out to ya'll too.

Does anyone on here have the Beckett Deeming waiver ? We've been turned

down three times for SS and I have an appt. with the DFACS office on Wed. to

discuss this waiver. Does the DMA-6 and the Care Plan need to be filled out by

specific diagnosis, what does that mean ? Any info would be appreciated .

Hi

We have the Deeming Waiver. Our doctor helped us fill out the DMA 6 and the care

plan we used was the the IFSP from EI. At the time all we had was a dx of DD

and nothing further. Actually, I pretty much filled out everything myself and

the doctor signed it. Feel free to call me at 678-715-0850. I can give you all

the details about everything. Our approval came through right before Christmas.

Plus anything else you have - like evals and stuff is helpful. Call me.
