Guest guest Posted January 9, 2002 Report Share Posted January 9, 2002 Hi We have the Deeming Waiver. Our doctor helped us fill out the DMA 6 and the care plan we used was the the IFSP from EI. At the time all we had was a dx of DD and nothing further. Actually, I pretty much filled out everything myself and the doctor signed it. Feel free to call me at 678-715-0850. I can give you all the details about everything. Our approval came through right before Christmas. Plus anything else you have - like evals and stuff is helpful. Call me.
