Matt,

I think I recall someone on here that was taking 50 mg, and doing well.

I personally take 90 mg and have for about a year or so.

my doctor wanted to increase my daily dosage of

Sprycel from 70 mg to 100 mg per day. While I

log increase, but that was while I was on Tasigna and

However, he was disappointed that we went from PCRU

to two successive PCRs showing increases. However, I

had to take a break on Tasigna as my blood pressure was

hitting 160/120 and I knew it was the medication, which

he disputed as I had gained a lot of weight on Gleevec.

Once I got on Sprycel, my BP rocked down as I had

projected to 126/76 which is more normal for me.

So this long story, to get my question out is this: If you

are on or have been on Sprycel, what was your normal

dosage? Was it 100 mg or are there people that are or

were on lower doses?

Hi Matt,

I am on what is considered to be the optimal dosage of Sprycel which is 100 mg.

However, it isn't just the amount, it's also a matter of when to take it. I

take all 100 mg in the morning which is what is recommended.

I did have one bout with pleural effusion early on but that was taken care of by

going off Sprycel for 3 weeks and tasking 40 mg of lasix per day. I now take a

maintenance dose of 20 mg lasix and don't have a problem. I did have about 30

pounds weight gain on Sprycel which was also a problem on Gleevec until I lost

my response to that drug and lost all the weight, too. I've been on Sprycel in

a clinical trial at the University of land since early 2004.

Matt,

My grandma is taking 50mg..she is stable at this point..but the doctors want to

increase it.  She just doesn't tolerate the side effects well on a higher dose

so is reluctant to increase it.

Thank you all for your comments,

They are all very helpful. When I

was first diagnosed, I wanted the

highest dose I could stand, but

Tracey helped me see that a

lower dose with fewer side effects

was better for your quality of life.

HI MATT, WHEN I STARTED THE BMS TRIAL FOR SPRYCEL I WAS PUT ON 70MG, DAILY.  NO

PROBLEMS, IT WAS GREAT, I REACHED CCR IN 10 MONTHS, FOR THE FIRST TIME EVER. 

BUT AFTER 1 1/2 YRS I HAD PE AND HAD TO QUIT.  SO, YES, I STARTED LOWER THAN

Matt,

I am on Sprycel(100mg).I started there and have reached PCRU in less than 4

months.I took Gleevec 400mg and 600mg prior to Sprycel for 15 months and never

reached PCRU.

I am also on Sprycel(100mg).I was not told to take it in the morning and have

been taking it at 5:30 every evening..I also gained 30 pounds in 3 months since

taking it.I lost weight on Gleevec.I also take a maintenance dose of Lasix as

both these meds have caused water retention in me.Do you think I need to switch

the meds to the morning?

Sharon,

No, I don't think you need to change to the morning. What I meant to say is to

take the full dosage regardless of what time of day you take it.

I think the normal starting dose now for Sprycel is 100mg once a day, and then

it can be dropped for people if their blood counts are too low or some other

side effect is a problem.....and then pcr followed to see if it is enough. (When

a was put on 70mg, she was in a trial of escalating doses and then just

stayed there because it was effective for her). Sprycel does not actually have

a therapeutic dose like Gleevec does. Because Sprycel is so much stronger, some

people are on 40-50mg and it works for them.....but the starting dose now is

100mg.

If I were you, I would go up to 100mg, see if you get your old pcru status

back....and then if your side effects are bad, see if you can drop the dose and

still maintain. That is what I see the specialists doing from my end.

I am a Dr. Druker patient and I am in a Sprycel trial, and started at 100mg for

3 years....then had some pleural effusion and we have gone back and forth

between 80 and 100mg because of PE (and I may end up switching to Tasigna)

because the 80mg dose is not really therapeutic for me.

I am also a patient who is on 100mg of Sprycel. I was on Gleevec for 18 months

and it never got me to where I wanted to be.

Last April, I was switched from 600mg of Gleevec to 100mg of Sprycel and within

6 months, I was in remission! Whoo hoo! It was also never recommended to me to

take the medication in the morning, just as long as you take it the same time

every day. I take mine in the evening also usually when I have dinner (that’s

helps me remember ;) ) From the medication, I have minimal side effects to what

I had with Gleevec but I have seen a tremendous weight increase on Sprycel when

I was losing while on Gleevec.

Out of curiosity, has anyone experienced a difference in their skin pigment

since taking Sprycel? I’m half Italian so I could be in the sun for hours upon

hours and never burn but ever since I started, even an hour in the sun burns me

like a tomato!

Jolie,

I am Cherokee and French and always was very dark. Not quiet as dark any more

But I have learned if I get a little sun at a time I tan and not burn. Yes I

have burned in the beginning days of chemo until I learned to treat my skin as

if I was fare complected. By getting small amts several times a day i was able

to build up and able to be in the sun all day without burning.

I am under the impression that the chemo affects the melatonin.

Matt,

After diagnosis in March 2007 and after discovering I couldn't tolerate Gleevic

I was put on Sprycel at 100mg daily. It didn't take long before I was

experiencing breathing problems and extreme fatigue. I was lowered to 50 mg

daily and slowly tried to increase and got to 70 mg when I once again had to

take a medication vacation because of side effects. I am now taking 50 mg every

other day and maintaining a great response. I have heard of some that are on as

low as 20 mg daily. I should add that Sprycel got me to PCRU within 3 months and

I have managed to stay at PCRU or at a trace but unquantative since October '07.

When I went to MD in July '09 and met with Dr. Cortez he said the key

is to find the lowest dose that will still keep you at a major response and also

provides a good quality of life. Everyone is different but there is nothing

that says you must stay at 100 mg. Dr. Cortez is one of the leading CML

specialists and even he confirmed that the dose is specific to the individual

and can vary from 100 mg daily to as low as 20 mg daily.(he has some patients at

that low dose)

Yes, I heard the same thing about not cutting the tablets. I take one 50 mg

pill in the a.m. and another at bedtime. The doctor thought I would end up

with fewer side effects if I didn't take 100 mg. all at once.

Just so you know, the general feeling among CML specialists and also on

clinical trials is that taking 100 mg all at once is the preferred method of

dosage because there are fewer side effects. I know that dividing the dose

may be working for you but it doesn't for most people.
