Between my two immune compromised boys, we have dealt with cellulitis 9

times and been hospitalized for it twice... because they have neutropenia

and a neutrophil chemotaxis defect, they don't usually have anything to

culture (no pus!) - J had a bit of swelling when it was his knee,

though-they thought he might have a joint effusion. Were they able to

culture it at all? (Not thinking so..but just wondering) With possible

MRSA, did they give an injection of antibiotics? Wish I had answers for

you. Thinking of you and praying the antibiotics help.

Peace be with you,

Pattie

Subject: cellulitis question

Hi Everyone,

My non PIDD child (who is 5 1/2) woke up with a golf ball size lump that was

slightly warm, red, and obviously swollen on her right arm this morning.

There was nothing there the night before. We took her to the ER and she was

diagnosed with possible MRSA cellulitis. There is no opening to the

cellulitis, so supposedly she is not contagious in any way. This of course

concerns me though.

Since I know the Moms and Dads in this group are experts at the " unusual " ,

I wanted to ask if anyone else has experienced major swelling with

cellulitis. My PIDD child has has cellulitis twice and neither one looked

anything like this and she required IV antibiotics. I don't know if MRSA

makes it different or not. Any thoughts would be appreciated.

Thanks,

Absesses and cellulitis may be linked to certain medical conditions such as PID

and metabolic disorders. I'd make sure the docs knows, take photos, etc.

There is another mom in our group whose kids have this problem...perhaps she'll

weigh in on this when she has time.

My 6 year old (probably) has Hyper IgE - he gets cellulitus a lot - from bug

bites, immunizations, red ant bites on his feet... and usually oral

clindamyacin is enough, but every once and a while we end up in the hospital

for IV antibiotics. We circle the site with a marker, which is usually red,

angry looking, and sometimes has a brown/yellow center spot. It is usually

too tender to touch but I am told on xray they can see a lump in there. If

the site doesn't grow too much then oral antibiotics are our route. If it

doubles in size quickly or is bigger than 2 inches in diameter we head to

emerg.

We have never been tested for MRSA so I can't help you there.

Hi Pattie,

The could not culture it because there was no open wound and they did not

want to open it. They did not give her a shot by they have loaded her with

Bactrim. We are watching her closely because it is hard to tell if it is

getting any better.

T would make me nervous not being able to tell if it is getting better.

We've always been able to tell.. Even with oral antibiotics.

We thought it was a spider bite, but the ER thought it was a tiny scratch you

can not see.

It's interesting that you say that it can be related to other things. I brought

up to my husband last night that it seemed strange that this is not my child

with PID. If we had known about PID when my oldest was little we would have

headed to an immunologist. She fit the transient diagnosis really well for

about a year and a half and has done well since. I always keep my eyes open

with her just to make sure we don't need to have her immunoglobulin levels

tested.

We have been taking some photos to watch the progression. Unfortunately, if she

keeps getting this slow redness spread we will be in the ER again tonight or

tomorrow. I think the Bactrim is failing.

Is two inches a magic number for cellulitus? She does have the center spot

which looks almost purple and they did say they could see the swelling on the

xray.

BTW : I wanted to say thank you to everyone who has been emailing on this

topic. It has been very helpful to learn from your experiences.

My husband was home with her today and he thought it looked better. I had him

take pictures and email them to me. I thought it did not look better. When I

got home from work I looked at it and called the doctor. It is definitely

spreading from this morning and she still has a low grade fever. I did another

eye liner pencil mark to see if it keeps growing and if it does we get to visit

the ER tonight or tomorrow. We are in the middle of a foot of snow being

dropped in the area so it is going to be an interesting trip to the ER if we

have to take it.

2 inches is just my personal guideline - but definitely circle it and if it

is bigger in the morning then the antibiotics she is on are NOT working....

it should be staying the same at least - definitely not getting bigger. The

problem is if it keeps getting bigger they worry about it becoming an abcess

and would look at draining it (surgically). They don't want the infection

to hit the bone ! Good luck and I'll be thinking about you in the

morning... we have the snow here too.

Redness right at the site can also be how it is healing, so I would mark the

outer edge of the redness to see if it's growing. The wound being warm to touch

is also another sign of cellulitis along with any kind of fever.

May be an infection. Put a circle around it and if it gets bigger redder etc go

to docs and get an antibiotic. My daughter has had cellulitis several times.

Can be very swollen, red and hot. U will k ow if it doesn't go away if she needs

rx. Just don't want it to go until blood stream

Also, know that if it is cellulitis, it can spread quite a bit before ever

getting a fever.  So, watch it closely.  My daughter is just getting over her

first cellulitis infection, and it got pretty big and pretty sore before I knew

it was there, but no fever.  I agree with drawing the line around it too.  That

will help you know whether it's spreading or not.
