Hello All!

I received an email from the transplant Dr. in Hamilton, it seems that after

reviewing my file and speaking with Dr. Lipton (the specialist I met with in

Toronto in Jan.) he has decided that my case is " urgent " after all and I will be

headed to transplant as soon as they can find a suitable donor...

I thought maybe we could have put this off for awhile since my pcr was so good,

however, he feels regardless of the good results now is the time to proceed...I

haven't heard from my local Dr. yet as to whether she will still be doing a

paper on my case for the hematological conference, but now that we are

proceeding this way, I guess it doesn't much matter.

So, I'm asking for good thoughts and prayers as I continue on this ride,

hopefully, moving toward being cured...

I guess this is a silly question, but if your pcr is good, what is the urgency?

Hello All!

I received an email from the transplant Dr. in Hamilton, it seems that after

reviewing my file and speaking with Dr. Lipton (the specialist I met with in

Toronto in Jan.) he has decided that my case is " urgent " after all and I will be

headed to transplant as soon as they can find a suitable donor...

I thought maybe we could have put this off for awhile since my pcr was so good,

however, he feels regardless of the good results now is the time to proceed...I

haven't heard from my local Dr. yet as to whether she will still be doing a

paper on my case for the hematological conference, but now that we are

proceeding this way, I guess it doesn't much matter.

So, I'm asking for good thoughts and prayers as I continue on this ride,

hopefully, moving toward being cured...

That is a question I have too - but I'm assuming that they want to proceed while

I'm in remission so as to have the best chance for a successful transplant...my

case is different in the fact that I presented in blast crisis - the protocal

for that is to get the person to remission and then transplant...

I still beg to differ with you and your Doctors.

I was in Blast crisis when diagnosed and here I am 3 years this week with no

sign of active CML cells and living a wonderful happy life.

I was told beings I was in blast stage upon diagnosis I would not make 2 years.

That was what they new about CML before all the new drugs have come out.

Now the Dr's have no clue what should be protocol.

Sprycel was FDA approved after I was on it. And there are more drugs in line

still.

It is your life and you can allow the Dr's to do what they want to with it or

you can take hold and make your own decisions.

Cheryl, I thought I read , maybe on Jerry's site, that your first diagnosis of

blast crisis was in error.  Maybe I'm mixed up.  But I would think they would

wait and see how long your pcr stays good before they go for transplant.  I am

wishing the very best for you, but truly wish you did not need to have the

transplant.  Have you emailed other experts such at Dr. Druker or Dr. Talpaz

Hi Cheryl: May the Lord be with you with whatever decision you make.

I have all the same questions that a has. Did you go for a few consults

regarding this BMT, or is this the only doctor who suggests it. I went for

numerous opinions, and they all told me BMT even with no sibling match. I chose

not to do it, but that was my choice.

I have posted to you before, and I was diagnosed in 1998. I was not in blast

crisis at any point. I am still on my 400mg. Gleevec Daily.

Keep us all posted. So many prayers are going out for you.

Hi Cheyrl,

I would just like to share my experience with you. I posted earlier about

relapsing after having a SCT in Oct, 2004. When I relapsed it was with

Extramedullary CML (tumorized). That diagnosis is considered to be in blast

crisis. The first thing I was told by my onc and transplant doctor at MD

was that I need another transplant, even though I was ravaged with

chronic GVHD. I asked about drug treatment and they both agreed that it's not

what they would recommend.

After a lot of thought and prayer, I decided not to do the transplant but to

take another TKI (Sprycel) -- both docs were not happy. Neither doc thought

that it would be effective and I wouldn't make it another month without a

transplant.

After 3 weeks of Sprycel treatment, the tumors were shrinking (PET scan

results). After six weeks they totally disappeared. A BMB revealed that I was

PCRU, the first time since transplant. I have been tumor free and PCRU since

April of 2006. I'm tested by PET and BMB quarterly. I take 60mg of Sprycel

daily with no noticeable side effects. Now would this work for everyone, who

knows. Our chemistry is so different that the outcome for some other person

could have been very different.

I had to make the exact same decision you have to make. When I was on Nilotinib

prior to transplant, it was working wonderfully, but again, I had been in blast

crisis when I started it and it was such a new drug (Phase I) the docs had no

idea if I would retain the remission.

I still wonder today if I would have foregone the transplant and stayed on

Nilotinib if I would have been better off. Just no way to see into the future

is there.

Please don't think I'm trying to talk you out of a transplant that might be your

only option, but I did want to give you another perspective. The decision is

yours, and should be. Gather all the knowledge that you can so you can feel

good about your final decision -- something I neglected to do.

Yes I have seen 3 doctors now who all agree that the best and only option for me

is the BMT...I know that that stirs up alot of emotions on this board, and I did

not post it for that reason. I just wanted to keep all of you updated on my

status since you have all been wonderful at providing me with a wealth of

helpful information since I started my cml journey last November...

There had been some speculation that the diagnosis was wrong, however, I have

had it confirmed my 3 doctors now - and they are all in agreement that BMT is

the way to go - since my case has not presented normally from the get-go , they

cannot be sure how long I will remain in remission on Gleevec...they are

currently searching for a donor and as soon as one is found I will be headed to

Hamilton Ontario to have the transplant... I cannot risk going into blast crisis

again as the morbidity and mortality rates for transplant after a second blast

crisis are significantly lower than if we do the transplant now...although it

has been a difficult decision to make, I know it provides me the best possible

way to be CML free - it will be a long and possibly difficult journey, but one

that has to be done if I want the best chances of survival to see my children

finish growing up and continue life as I know it...

Cheryl, I wish you the very best. Your decision is not one that I

would like to face and everyone on this list is wishing you the very

best. Even though there are many wonderful new medicines for CML,

your case highlights the fact that we still don't have a CURE. May

God bless and protect you in this difficult time.

Druker? If not, I think that it would be a good idea to do so and ask what his

Hi Cheryl:

 

I am 3 years post transplant for CML.  I presumed the same as you about waiting

to see where it would head but I was told to go to transplant as soon as

possible when the decision by my doctors was made.  At first I wanted to wait

for at least six months but the doctors said blast crisis could happen overnight

and then it would be to late for the transplant and I would have lost the window

of opprotunity.

 

The good news is that I saw my doctor in Atlanta last week and my cells and

blood are perfectly normal.  He told me this transplant couldn't have been

planned out any better than it turned out.  I am glad I actioned sooner than

later.  If I had waited I wouldn't be hear to tell you my story.  Once you

blast you will lose your window of opprotunity.

 

Transplant is a hard journey and everyone has a different path as they go

through it.  I had a drill sargent caregiver who kept me going.  Also I signed

up for a trial chemo regime.  They gave me a different combo of chemo drugs to

try to help minimize the GVHD.  It worked very well and I had a small fight

with GVHD and eventually got back on my feet.  I am now back to working full

time and getting on with my life.  Let us know where your path takes you, I

will add you to my prayer list.

cannot be sure how long I will remain in remission on Gleevec...they are

currently searching for a donor and as soon as one is found I will be headed to

Hamilton Ontario to have the transplant.. . I cannot risk going into blast

crisis again as the morbidity and mortality rates for transplant after a second

blast crisis are significantly lower than if we do the transplant now...although

it has been a difficult decision to make, I know it provides me the best

possible way to be CML free - it will be a long and possibly difficult journey,

but one that has to be done if I want the best chances of survival to see my

children finish growing up and continue life as I know it...

Thank you Therese for sharing your story with me...it is a hard road to take,

but one that I am prepared to walk down...I know that it is the only option for

me...it is hearing from people like you that makes me know that I made the right

decision...

Dear Cheryl,

What a cloud hanging over you!   We do hope that you have good family support.

Sometimes the worst torture is the waiting.

I think your docs would not do a transplant if they were not optimistic.  Just

think of all the benefits that the future holds for you and for your loved ones

when this is behind you.

All your friends at are rooting for you and we hope we can be advised of

your progress from time to time.

Yes I have seen 3 doctors now who all agree that the best and only option for me

is the BMT...I know that that stirs up alot of emotions on this board, and I did

not post it for that reason. I just wanted to keep all of you updated on my

status since you have all been wonderful at providing me with a wealth of

helpful information since I started my cml journey last November...
