Guest guest Posted December 14, 2007 Report Share Posted December 14, 2007 Hi Philip, My wife has been receiving both E and T pellets, 3 and 2 respectively, since 1991 as these were considered by her OB/GYN Endocrinologist to be the essential hormones in the same dose and same balance as before menopause. She was restored to being a happy loving wife. In recent years women have become aware from other women writing on the subect that T levels in women are never measured (well almost never) like they don't test of low E for a woman suffering from menopause, they just write the script for E only. I was surprised to hear that between 10% and 20% of women can not metabolize the oral medications and get no benefit because blood levels are never tested (almost never) to see if the woman is in that group. That is where pellets saved me wife. Those in that group never know they are there, they just take the meds and feel miserable. There was one OB/GYN in Rochester, NY that had only one other patient interested in T pellets prior to my wife who needed to find another doctor after Dr. Sipi passed away. She wanted them for libido and he gave her one or two 25 mg pellets just as an experiment to see what would happen. It is very uncommon to find a doctor that will provide both T and E to a menopause women. She was talking to her PCP that gives her the B12 he did a blood test for T and added 50mg of T to the shot. What a differance that made in her, she is almost like a 20 year old again. She had a histromecty(ms)in 1982 and has been on female harmone ever science. Her GB-Oyn did not know how (or would not take the time) to treat her low T. Now her PCP gives her her female harmone in patches and her B12 and T in shots. Some time we go looking for a special Dr to treat something and skip our PCP. We both use the same PCP and he has taken over my T shots and does a better job than the Uro. With my last T blood test I asked him for an E2 test. He said ok, I asked do you know what that is in a male, he said no but you have researched it so lets find out what is is. E2 turned out to be 24. We have over a 10 year relationship with this PCP and he seems more flexable and ready to help than any other Dr. we have been to. Buy the way DO NOT PUT ANDROGEL ON THE CLIT OR PENIS, read the package insert it says do not apply to the genetal area. I was on ineffective gels for years. As for your wife this is great but I wonder in the long run if this is good for her. My Wifes cousin was dam sick and a teacher she had to go on disability. She went from one Dr. to the other until she found a Dr. that told her he could give her pellets what the hormones in them that she needs. Wow she go so much better that she went back to collage and then ended up the principle of the school she worked at. This Dr. told her shots were not good for women can't remember why. Anyway her big problem was she went into her change and nothing helped not one of the Dr.'s she seen ever checked her Testosterone. She has been doing the pellets now for 20 yrs. And has no trouble finding a Dr. to do them she gets her pellets made at Collage Py. and they last 4 months. I was aware T keeps bones healthy, and of course females need T for that purpose, as we need some E. I was also aware " estratest " a female hormone replacement, contains an adequate amount of T for females, and it improves their libido. It does not take a rocket scientist to know, literally thousands of women suffer bone loss as they grow older, because noone puts the word out about their need for T. In my opinion, here in the USA, our puritan views about sex, has it's negative down side, good god, deacon jones, don't talk or learn anything about hormones, you sinful bastard. My uro has confessed, he went to med school in late eighties, and only had a two hour movie on hormones in his training, said it was a saturday and he slept through it. I use DMSO and injectable T mixed to massage my SO's back, within hours it turns her on. Pretty kewl. T is also essential for females to have a healthy libido. Again in my opinion, women need to have balanced hormones, as we men need. Another area of darkness, is that females have less than forty eight hours of the neuro-transmitter, serotonin in store, whereas males have about seven days supply stored for use. Serotonin is the NT, that gives us a feeling of well being, energy and that periodic feeling, " its great to be alive " . For our bodies to make serotonin, we have to eat something, meat, preferably turkey, hi in the amino acid, (I think there are twenty amino acids that make up protein,)trytophan in it, and it should be eaten with carbo's, to get the trytophan into our brains, (and trytophan does not compete well with other amino acids to cross the brain barrier), so we can then make serotonin. Is it any wonder, we have so many confused and emotionally disturbed females today, with diets restricting carbo's. And generally speaking, it has been my experience to find that the thinner the woman, the more crazy she may be. She started the b12 and T shots about 2.5 years ago. She is still on the female patch and has been on female harmone for 25 years with no ill effects. Our PCP was the one that figured out she needed the T and she was resistant to it at first took her almost a year to make up her mind to do the T. She is 2.5 years older than I am but can run circles around me. To my knowledge there is no Dr here that does pellets. In October just before my failed implant surgery Uro said something about a new pellets for T available in 9 month or so that would last for 3-4 months, but I beleive he is talking about the new long lasting T shots. Female harmone has been good for her for 25 years and going. Last year DR tried to take her off the patch MISTAKE!!!!. A month later I went with her to Dr and he instantally put her back on the patch. Lots of luck to your and your wife. I wish more Drs were like ours. I am going the the Dr that is Head of the Urology Department for The University Of Tennessee (Memphis) Jan 9 for a second opion for an Implant. Will ask him about pellets. My T shots are 300mg every 10 days. If I go 3 weeks without a T shot my T drops to 120 Range 241- 827. I drop by the Dr office every 10 days and Medicare and my supplement pay the whold bill of $135.00. Good luck to you and your wife. She sounds like a gem for you. The wife complained she was not getting any benefit and suffered for 1 anf 1/2 years. He even suggested it was mental and needed to see a psychiatrist instead of testing her blood level for E. He blamed the patient and she suffered needlessly. When a friend saw an article about a similar woman in the Saratoga Springs news paper, she went to the same doctor and got pellets. She was a different lovable woman in a week. she did get a booster shot of E in the same visit. I got my endocrinologist to guess and he gave me a range of women that may not be able to metabolize oral medications for E and I imagine the same can be true of oral meds for T. He estimated between 10% and 15% of women may not be able to metabolize the oral meds into hormone in the blood stream. The comments about the effect on the clit may be over stated but when she went on the pellet therapy, she felt there was lots more sensitivity than before. It would become erect more frequently but did not enlarge significantly. Self administered SubQ in the belly they are painless and take about one minute of my day to administer, and my dose can be adjusted on a moment's notice if needed. That seems like far less hassle than your long distance trips to Georgia to get pellets implanted. And since many of us don't have relatives in GA to visit as part of the justification of the trip, your perspective on pellets is a bit skewered from the norm. ;-) I agree, if patients were more demanding of the treatments they want to try, doctors might be more willing to try them, but honestly, pellets do not seem compelling enough to me, or more convenient, less painful, or any other way more advantageous than bi-weekly shots. But we are each different with different needs and priorities, and what works for some won't work for others. I have been doing the shots once a week in the thigh with a 22 x 1 needle, for 17 years. Lets be real about this, some men are just wimps about injecting themselves and want the smallest needle, and to avoid doing the shots IM as they were meant to be. Whatever works I guess, but this method has worked very well for me and I keep my level at about 3/4ths the scale with no E2 problems. I have watched the commits on this group for a while now and if there is a magic bullet about TRT, I have failed to find it reported on here. Some of us just do what we need to do............... My major reservation about pellets is the inability to tweak levels once they're in. Recently I found that my T levels jumped from about 700 to 1100 because I lost 25 pounds. I simply knocked the dose down and things were level in a week or two. With pellets a surprise like that is a long term affair to adjust. We've all seen here we absorb at different rates and can reach vastly different levels on similar doses. I feel that fine tuning possibility a big plus for shots. I did IM for a long time, but it was the Doc's suggestion that we try SubQ. Shippen is now recommending SubQ and more frequent dosing as a method to reduce spike that cause high E2. I think Dr (Chrisler) won't be too far behind in those recommendations as well. SubQ is less painful, but IM isn't that bad once you get used to it either. But I think anyone who has a choice will choose the less painful method, especially if it might have added benefits (lower E2 conversion). I'm surprised at your total recall about the logistics to get to my doctor. I'll give you an update. Dr Gambrell passed away in June this year and now his associate, Dr. Natrajan cares for my pellets. We moved from NY in Dec 2005 to be closer to family and now live in Durham, NC so the total trip takes a long day over and back to Augusta, GA. I have enjoyed the pellets so much I have been trying to help others be aware of this form of therapy. Maybe patients demand will make a difference. Much to my surprise other parts of the US do not have the same benefit from Medicare. I pay about $60/visit. If I had to pay it myself it would cost about $450/visit three times a year or $1350/year. Gasoline for 800 miles for each round trip is another cost for me. His reply that you might now be at a higher level than 1000 is not a serious problem due to the gradual changes due to lost weight. He mentioned patients are sometimes given high doses of T as a way of building up the blood for patients with even higher doses than he provides but causes no harm. He also mentioned another condition that is treated with high T but can't remember what it was. High T is not toxic. It could be adjusted with the next set of pellets with no problem. Have you heard of this before? The variation in patients absorption with creams and ointments seems to be a problem. Some times pellets get into a fatty area where the blood circulation is not good and the benefit is not realized because the pellets dissolve too slow, but they do finally dissolve.
