In regard to getting the information toy our insurance company the DR's office should be able to forward that to you. At your neuro visit if he gives you a prescription for the band- YOU MUST HAVE ONE TO GET A BAND-also ask him for a copy of his progress notes from your visits and a letter of medical necessity from him. He should be able to provide all of this to you. Then make plenty of copies and then take it to wherever you are getting the band made. They should file it all for you with your insurance and work out the preapproval so you know what is covered before the rest of the therapy continues. Insurance is very confusing. Attached is a link that Aetna has provided a statement regarding banding- hope it is helpful for you.
