I have only had experience with one cast that got too tight, and I don't yet know what they are supposed to look like long-term since we are only in our second cast. I know there are some regulations that doctors aren't supposed to communicate via email regarding specific medical stuff anymore and perhaps that is why you did not hear back. Dr. K used to be awesome about emailing me and then he was told due to some regulations (I forget what he said they were called) he couldn't correspond with me that way anymore. Aack!! The pointy parts that control the rotation are so low. It doesn't look like the normal mushroom cutouts. Is his curve lumbar rather than thoracic? He did have a rib bump this last time and Dr. S was concerned and said we really need to watch for rib deformity. Maybe that contributed to the cutouts? If they don't read e-mail, FED-EX overnight those pictures to the doctor. Make sure they know that Jack is not eating!! Maybe also call tomorrow - make sure she sees the pictures. It would expose a little more and not so uncomfortable for him. If you can do that then its better to keep him in it growth is good. I was wondering why his mushroom cutout is large at the top and narrow at the bottom. My son Dylan's cast is narrow at the top wide at the bottom. (upside down mushroom) The wings on the sides are directly over his lower ribs. What exactly did your Dr. call your cast. I've heard so many different names for cast's or techniques. Dylans now in his six cast everyone of them has been different same concept but different. His first five Dr. called them Localiser casts done on a spica table. The cast he is in now is a Risser Cast done in SLC. I have looked at so many pictures of everyones kids and there's so many makes and models its like buying a car. I hope all goes well. It truly looks like it's pinching him on that one side especially... I " d get in touch w/ someone ASAP. You're on top of things and will get it right, but make sure someone gets him scheduled for a cast change as soon as they can so he won't have to be out of the cast and lose any correction achived. I had a lot of people look at the cast last night and many said, " Well, it looks tight - but I don't know how it's SUPPOSED to look, kwim? " So, it helps tremendously to hear other casting parents say it's too tight. I emailed the pictures to the doctor last night and am anxiously awaiting a reply. DH will follow-up with a phone call today. My frustration with this facility is coming to a head. It's time to have someone local - period. Jack deserves more available care than this! It's literally been weeks of us trying to get in contact with someone up there and having just a scheduler call us back with dates. I'm fed up! Darrell, For insurance and hospital forms, they refer to them as Risser casts. However, we like to refer to it as a Mehta-style cast (since she's the originator of this design). The cut-outs are all different because the curves are all different. Even Jack has had two different types of shoulder straps - I think just because they do whatever in the OR works. He has a very large curve (seventy degrees on diagnosis) with significant rotation of the spine - pushing his ribs out on one side and hollowing his back out on the other side. Miss Mehta did his first cast and the others have looked very similar, but this one does not have the windowpane in the back. Now as for the appearance of his cast, in Chicago has developed a way of petaling that involves cutting the t-shirt underneath, padding the edge of the fiberglass with a cotton padding, and then taping it to the front. She then recommends covering it in duct tape. She's very experienced in casting and went to school for it (I believe in Massachusetts) and we just adore her. All of Jack's casts have come back looking really clean she said, in comparison to other castings she's worked with (not necessarily spine) and with the tee shirt being attached to the cast itself, it allows us to see his body and clean him more thoroughly when we give him a sponge bath. I really like this method of " petaling " best. Oh - and last month he had a tummy bug that caused him to have some explosive/leaking diapers. I was able to un-tape the back, stretch the tee shirt down, cut off the lowest part of the tee shirt, and tape it back on. (And excuse the Nemo stickers - he had some scrapbooking stickers all over his cast in the early days, but they've come off since and he just has pearl and the blowfish left) Sandi Quote Link to comment Share on other sites More sharing options...

With the shirt not stapled to the cast, it was harder for me to diaper. I took to taping the shirt to the cast. But, the advantage to the shirt not being stapled and petaled to the cast is that you can pull it down (they are super stretchy) if poop or pee gets up there and clean it with a sprayer and then blow dry it. The antibacterial feature is nice because it cuts down on the germs of course, but also because the cast is less stinky...definitely not stink-free, lol, but it was definitely less stinky than that orginial cast. And you even said you have to drive to Chicago, that will not be a ride you would want to take with a child in pain. When I saw the pictures, I just cringed. I would really get that cast off. I'm thinking of hooking up a blow-dryer at the diaper station to dry it out each time. LUVS has straps on the side - maybe that contributes. Are there other diapers that are gentler on the sides when tucked under? Pull-ups look gentler on the sides, but they probably won't work on a 17 month old, will they? They are a pull-on diaper for littler babies/toddlers who don't want to lay down for a change. Maybe those would work! Right now we are just really careful about flattening out the interior tabs of his diapers and we put it snug on him. I did use the blowdryer on cold at diaper changes as well. I also applied the 1% hydracortisone each diaper change unless it got flared up and had the blisters, which was an indicator of the skin breakdown, then I used the bactroban. Guest guest Posted January 18, 2008 Report Share Posted January 18, 2008 After a whirlwind afternoon, Sophia is out of her cast. It was run, run, run back and forth across town but we were able to get her into both the hospital to cut off the cast and into the Pediatrician to evaluate her skin. Her doc said there was no way she could have gone another week in that cast. She has gained 1.5 pounds in the last 3 weeks- more than she gained in the previous 3 1/2 months!! So we definitely had a growth spurt, and I am anxious to get x-rays and see if we have any correction. They have us using Neosporin on the areas where the skin was broken, but no oral antibiotics unless it gets worse. The side of the cast at the bottom was worse than her hips, but we couldn't see it with the cast on. It was literally raw, so glad we have 10 days to heal up before next casting. We are praying that won't do any harm or lose any correction. Dr. Strum said last time that one week shouldn't hurt. Thanks for the kindness and encouragement! We also talked to today and she gave us the okay to get Dylan's cast off as well to let the skin heal. Our ped. swabbed the skin to make sure it wasn't a bacterial infection (it'll take a cpl days for the culture results to get back), but she was pretty confident it was nothing more than irritation or possibly a little bit of a fungal infection from sweat or urine getting on that area near the diaper. She gave us a prescription for a fungal cream and told us to use hydrocortisone cream and/or vaseline on the skin for the next few days. She thought it was good we got the cast off now to let the skin heal before the next cast. She also thought he had definitely outgrown this one. Now I just hope Sandi and can get some answers very soon. I can't believe didn't call you today?!? It seemed that she was kind of catching up on calls (with the one to me & then to ). I thought that perhaps she had been out of the office again or something. I don't blame you for being frustrated. Did you try to have her paged? Try to speak with again? That is crazy that you are getting no response after weeks of calls and e-mails. I didn't get any pics unfortunately because everything happened so quickly today. Her skin is already looking so much better tonight after her bath and some of the cream Dr. Metha told us to use (calendula). I have two pics of the spots on her hip bones that were taken 12/23 though, so you can imagine how much worse they were looking almost a month and 1.5 pounds later. I will add them to her photo album cast#4 on the group. Her belly button was kind of looking like Jacks, but instead of folded over, it was going under the cast partially. Let us know when you get a date from Chicago. We'll be there Sunday 1/27 at 2pm for x-rays, then Monday 6AM for outpatient same day surgery. I am praying they can get Dylan and Jack worked in then too. They will already have the equipment in the OR, so I would think it would be easier to move a couple other non-casting kids. Please keep us posted and enjoy your cast free hugs (I sure am!). I didn't get any pics unfortunately because everything happened so quickly today. Her skin is already looking so much better tonight after her bath and some of the cream Dr. Metha told us to use (calendula). I have two pics of the spots on her hip bones that were taken 12/23 though, so you can imagine how much worse they were looking almost a month and 1.5 pounds later. I will add them to her photo album cast#4 on the group. Her belly button was kind of looking like Jacks, but instead of folded over, it was going under the cast partially.
