Yes, , take that letter with you. Take all written copies

showing results of your EMG/NCV's, and other tests you've had, plus any

reports of surgeries, pain management therapy, doctors statements about

your medical leave, plus a list of all doctors you have seen, list the

reason, and the outcome. If you've seen a Physical Therapist or

Occupational Therapist, list their names and get copies of their

reports too. If your doctor suggested your medical leave from work,

somewhere that is documented. Get your doctor to write a letter

attesting to this, and how now you are UNABLE TO WORK.
