Has she been evaluated by a Mehta trained doc? Did they say she was a candidate for casting?

She has not been evaluated. I am worried about her respiratory status. I did contact Shriner's in Erie PA. They suggested Chicago.

We would have to fly there and she is on oxygen so that would be difficult.

If you are considering if she should be casted, then I would suggest having her evaluated by a Mehta trained doc. Other docs really can't say for sure about the Mehta method since the casts are different and have cutouts, etc. However, if she is on oxygen, she may not be a candidate. Also, if her scoliosis is not progressing, they may not want to risk putting her under general anesthesia (required for Mehta casting). Best wishes.

Sorry I have not updated lately- no news until now. But I do read most of the posts and pray for all of you and your children. I learn something from this site daily.

Lillyanna had her four month check up with Orthopedic surgeon on Friday. Her x-ray showed her at 40 degrees. She started at 44, then 36, now 40. He says it still is NOT progressing. He wants to see her again in 6 months. She is two years old and ways 19 pounds. She is on oxygen and has GI issues.

My dilemma is I have a good friend Orthro RN,NP that says this MD is the best in Ohio and I should believe him, he is never wrong.

I am worried that she is not a casting candidate yet, d/t all her other medical problems. But I am thinking of getting a 2nd opinion in Cincinnati?

Any ideas or comments??? Thanks.

I do not know of how much assistance the Cinncy Shriner's will be considering they do not actually do the casting there (too bad for us, huh?!) I know some people go there and that it is a top notch facility so maybe they will chime in with more knowledge than I have. It is hard to know what to do with the other issues you have to deal with. I wonder if that would make the casting difficult or impossible. Is that why Erie referred you to a different Shriners? I have been thinking of you. Please let us know what you decide to do.

**On a side note... My husband and I are going to the BIG GAME this weekend and I second your sentiments- GO BUCKS!

Hi ,

I can almost bet that the doc is not familiar, with the type of edf

(elongation, derotation, flexion)serial casting that we are all talking

about. Most docs today are familiar with old fashioned Risser casting

done on an old Risser frame, which does not have the ability to correct

the scoliosis 3 dimensionally. Risser casting is usually used as an

attempt at maintenance or post op fusion or hardware. Have you been able

to share with the current ortho surgeon Dr. Mehtas published paper in the

Journal of Bone & Joint (w/ the photo of he baby in a properly applied

jacket), or watch " A New Direction, " DVD, yet? These 2 items are

essential in understanding the principles of this gentle, corrective

treatment. The correct frame is also essential and an edf cast cannot be

applied properly, without it....

No doubt that her current doc is a skilled surgeon, and he/she shouldnt

mind that you are seeking out multiple opinions. Docs cant be expected to

know everything and early treatment w/ edf/mehta casting is very new in

the orthopaedic world. They are human and do make mistakes.

Unfortunately, 40 degrees is significant for a child so small. IMHO, 6

months until the next appt. is too long. There have been many children on

this group whose parents also felt (because they were advised by their

original doc) that early treatment w/ edf casting would not be an option

for their child. Probably, because the doc is only familiar with the old

way of casting, which they consider old school because tight casts done on

an inappropriate frame, with no windows are very restrictive,

uncomfortable and can cause chest wall deformity, and dont

correct.....These parents went with their intuition, made an appt. w/

doc's practicing the edf method, received proper treatment and now their

children are straight, or straighter than they were before.....

I have seen children with GI issues and oxygen receive and do well with

edf casting. The difference with an edf/mehta jacket is that there is a

wide, mushroom shaped window in the front (chest expansion window) for

proper breathing, a cut out in back to address rotation, and of course its

done on the right equipment. Old fashioned plaster casting doesnt address

rotation and this is key in training a young spine to grow straight.

I can provide to you the docs that I know of who are familiar and

currently applying serial edf jackets on their young patients in the U.S.,

if you wish.

at home in Denver. If you would like to discuss this further, i can be

As you know every childs scoliosis is very different, so maybe Lillyana is

not a candidate. Seeking out multiple opinions from qualified edf docs

precious darling.

Have you been given an RVAD, or indication of rotation? Do they x-ray her

standing with arms held in front, or over her head?

Thanks to everyone for their comments! Lillyannna has no head or trunk control so all her x-rays have been supine. I think Chicago Dr. Strum is the closest Mehta trained. I have the DVD of her x-rays except for this last one. I need to fill out the application on line and see how I could get her there. I also have two other medical fragile children so it is very difficult to get that far away. But I will work on it!

Hi !

There is a Shriners Hospital in Chicago and they offer help transporting people from other states. At least, the one in Phialdephia does. I met someone who is always transported from NY to Philly. Is Dr. Sturm at a Shriners Hospital? I would try to find out how they can help. The headquaters for Shriners is in Tampa, FL. If Chicago is not very welcome, try calling Tampa directly. You have nothing to lose.

I would get an opinion from a metha trained doctor btw what is the

RVAD just because she has not progressed does not mean she wont or

she may not but you have time NOW not later to cast.....I had THE

BEST ortho pod in the country I was told many of the metha doctors

trained UNDER him and he said casting was no good when we started

then after casting and him actually seeing the process recommends it

to others so stick with your gut and remember you have a small window

I think if we stuck to our gut and would be zero but we started at 30

degrees and by the time we were casted he was 38 and 23 months old he

was casted till 4 and now rotation free pretty much and stable with a

10 degree curve but where would we have been with out CAST group

anyway

get a 2nd and 3rd opinion from people who cast
