If you have not received it by your anniversary date, contact the office. You may have fallen between the cracks or it may have been lost in the mail. KEEP IN MIND...the office must have current contact info on us. If they don't have a current Mailing address, phone number & email address they can't do their part! We mailed a lab slip to you on March 1st 2007 and one went out March 5th of 2006. I didn't get them back in the mail. Please let me know if your address has changed.
