The term is used to describe a subset of children who have Obsessive Compulsive Disorder (OCD) and/or tic disorders such as Tourette's Syndrome, and in whom symptoms worsen following strep. infections such as "Strep throat" and Scarlet Fever. The children usually have dramatic, "overnight" onset of symptoms, including motor or vocal tics, obsessions, and/or compulsions. In addition to these symptoms, children may also become moody, irritable or show concerns about separating from parents or loved ones. This abrupt onset is generally preceeded by a Strep. throat infection. PANDAS is similar to that of Rheumatic Fever, an autoimmune disorder triggered by strep. throat infections. In every bacterial infection, the body produces antibodies against the invading bacteria, and the antibodies help eliminate the bacteria from the body. However in Rheumatic Fever, the antibodies mistakenly recognize and "attack" the heart valves, joints, and/or certain parts of the brain. This phenomenon is called "molecular mimicry", which means that proteins on the cell wall of the strep. bacteria are similar in some way to the proteins of the heart valve, joints, or brain One part of the brain that is affected in PANDAS is the Basal Ganglia, which is believed to be responsible for movement and behavior. Thus, the antibodies interact with the brain to cause tics and/or OCD clinicians use 5 diagnostic criteria for the diagnosis of PANDAS (see below). At the present time the clinical features of the illness are the only means of determining whether or not a child might have PANDAS. Q. What are the diagnostic criteria for PANDAS? A. They are: Presence of Obsessive-compulsive disorder and/or a tic disorder Pediatric onset of symptoms (age 3 years to puberty) Episodic course of symptom severity Association with group A Beta-hemolytic streptococcal infection (a positive throat culture for strep. or history of Scarlet Fever.) Association with neurological abnormalities (motoric hyperactivity, or adventitious movements, such as choreiform movements) Q. What is an episodic course of symptoms? A. Children with PANDAS seem to have dramatic ups and downs in their OCD and/or tic severity. Tics or OCD which are almost always present at a relatively consistent level do not represent an episodic course. Many kids with OCD or tics have good days and bad days, or even good weeks and bad weeks. However, patients with PANDAS have a very sudden onset or worsening of their symptoms, followed by a slow, gradual improvement. If they get another strep. infection, their symptoms suddenly worsen again. The increased symptom severity usually persists for at least several weeks, but may last for several months or longer. The tics or OCD then seem to gradually fade away, and the children often enjoy a few weeks or several months without problems. When they have another strep. throat infection the tics or OCD return just as suddenly and dramatically as they did previously. Q. Are there any other symptoms associated with PANDAS episodes? A. Yes. Children with PANDAS often experience one or more of the following symptoms in conjunction with their OCD and/or tics: ADHD symptoms (hyperactivity, inattention, fidgety) Separation anxiety (Child is "clingy" and has difficulty separating from his/her caregivers. For example, the child may not want to be in a different room in the house from his/her parents.) Mood changes (irritability, sadness, emotional lability) Sleep disturbance Night- time bed wetting and/or day- time urinary frequency Fine/gross motor changes (e.g. changes in handwriting) Joint pains here is another list from another site Red flagsThe biggest difference between regular OCD and PANDAS OCD is the sudden onset. OCD usually develops slowly over weeks, months or even years; however, PANDAS OCD seems to come out of the blue with parents saying one day their child woke up a different kid. There are also other symptoms that help differentiate the 2 types of OCD. PANDAS OCD is also characterized by: Choreiform movements: involuntary or irregular writhing movements of the legs, arm or face Presence of tics and/or hyperactivity Irritability, temper tantrums, or mood lability Nighttime difficulties Severe nightmares and new bedtime rituals or fears Separation anxiety Age regression: going back to younger developmental stage New handwriting problems, loss of math skills, sensory sensitivities Q. My child has had strep. throat before, and he has tics and/or OCD. Does that mean he has PANDAS? A. No. Many children have OCD and/or tics, and almost all school aged children get strep. throat at some point in their lives. In fact, the average grade-school student will have 2 – 3 strep. throat infections each year. PANDAS is considered when there is a very close relationship between the abrupt onset or worsening or OCD and/or tics, and a preceding strep. infection. If strep. is found in conjunction with two or three episodes of OCD/tics, then it may be that the child has PANDAS What to do if you suspect your child has PANDAS Contact your pediatrician. Advocate for your child by being clear about what you want your doctor to do. In some cases, you may have to find a sympathetic doctor who is willing to learn about PANDAS. The PANDAS Clinic at NIMH may be willing to consult with doctors. Have your pediatrician do a throat culture of group A B-hemolytic strep. Ask that the specimen be cultured, not just the rapid test. Your doctor may also do a blood test to assess elevated streptococcal antibodies (Anti-DNAse B and Antistreptolysin titers (ASO)). You and/or your doctor can get information and treatment guidelines from the National Institute of Mental Health (www.nih.gov); type in PANDAS to access the appropriate link. Contact the OC Foundation to help locate an expert in OCD in your area. Have your child retested if there is a resurgence in OCD symptoms. yes my son and best friend both have PANDAS< my son had scarlet fever and strep throat and deteriorated what felt like over night almost 2 years ago, which we have then seen a pattern with 3 other episodes, which gives him the dx of PANDAS, he gets very OCD over letter and turns extreamly hyperlexic, his hand writting goes, he has melt downs over nothings, lots of tantrums, gets mean and will hurt his sisters for no reason ect. my aspie friend well get really depressed, and emotional, have melt downs, becomes unfunctionable, her immune system crashes and she will over night new allergies to the place she can bearly eat. (she is an adult and it looks a bit different) she was previously dxed with tourettes which I pushed her to get tested as I saw it come and go like my son with his PANDAS flares and sure enough she tested possitive, so now with treatment OLE (olive leaf extract) she is tic free as well. she has the history of tons of strep infection as a child and being on spectrum as well but wasn't caught till this last year. so her peices are coming together now. The only strange part is she has not had strept throat in her life. We had a terrible case of the stomach flu with diaahrea and throwing up over the fourth of July. Since then abrina will not ride her bike anymore, she will stay on the porch rather than playing with her sisters in the driveway, sleeping on the floor instead of her bed. Very whiny. Always saying Mom come back and I am standing next to her. Crying alot for no reason. I thought it was the yeast back. Her DAN Dr put her back on diflucan and thought adding Li might help. She also recommended putting her back Adrenal Cortex might help. We had run out about 2 weeks before we saw the doctor in August. Now I am wondering if she has Strep. I have not given her any OLE in quite a while. Is there anything else I should be looking for in her and any other things that may kill it if it is Strep? You can also use cumanda. These products also treat lyme which a lo of ASD kids have as well....my kiddo has both. I am using from nutrimedix samento, cumanda and burbur. I am also going to order the carnivora. We have been doing IV chelation every week all year and my sons strep level (ASO titer) went from 267 to 132.....we have also been dumping a lot of aluminum. The strep and aluminum are bound to each other. My grandson, aged 10, ASD, high lead - has just been diagnosed with PANDAS and high Candida. He has had 2 courses of Penicillin for strep throat infection and also Nystatin for yeast. Will he always have PANDAS or can it be cured? He began OCD behaviours last year and is still starting new OCD - tapping his chest, noises in his throat etc. I personally think some of the first OCD behaviours have diminished a little but his parents are getting discouraged. He has been on chelation for 3 years and they think he is regressing somewhat. I wonder if anyone else who has a child with PANDAS has the same problem with probiotics with strep lines in them?? I've been wondering in the back of my mind about my asperger daughter having PANDAS. We tried Threelac earlier in the summer twice with a bad experience both times. But I attributed that to die-off, and I've been scared of trying it again since. April PANDAS, is an abbreviation for Pediatric Autoimmune Neuropsychiatric Disorders Associated with Streptococcal Infections. The term is used to describe a subset of children who have Obsessive Compulsive Disorder (OCD) and/or tic disorders such as Tourette's Syndrome, and in whom symptoms worsen following strep. infections such as "Strep throat" and Scarlet Fever. The children usually have dramatic, "overnight" onset of symptoms, including motor or vocal tics, obsessions, and/or compulsions. In addition to these symptoms, children may also become moody, irritable or show concerns about separating from parents or loved ones. This abrupt onset is generally preceeded by a Strep. throat infection. PANDAS is similar to that of Rheumatic Fever, an autoimmune disorder triggered by strep. throat infections. In every bacterial infection, the body produces antibodies against the invading bacteria, and the antibodies help eliminate the bacteria from the body. However in Rheumatic Fever, the antibodies mistakenly recognize and "attack" the heart valves, joints, and/or certain parts of the brain. This phenomenon is called "molecular mimicry", which means that proteins on the cell wall of the strep. bacteria are similar in some way to the proteins of the heart valve, joints, or brain One part of the brain that is affected in PANDAS is the Basal Ganglia, which is believed to be responsible for movement and behavior. Thus, the antibodies interact with the brain to cause tics and/or OCD clinicians use 5 diagnostic criteria for the diagnosis of PANDAS (see below). At the present time the clinical features of the illness are the only means of determining whether or not a child might have PANDAS. Q. What are the diagnostic criteria for PANDAS? A. They are: 1.. Presence of Obsessive-compulsive disorder and/or a tic disorder 2.. Pediatric onset of symptoms (age 3 years to puberty) 3.. Episodic course of symptom severity 4.. Many kids with OCD or tics have good days and bad days, or even good weeks and bad weeks. However, patients with PANDAS have a very sudden onset or worsening of their symptoms, followed by a slow, gradual improvement. If they get another strep. infection, their symptoms suddenly worsen again. The increased symptom severity usually persists for at least several weeks, but may last for several months or longer. The tics or OCD then seem to gradually fade away, and the children often enjoy a few weeks or several months without problems. When they have another strep. throat infection the tics or OCD return just as suddenly and dramatically as they did previously. Q. Are there any other symptoms associated with PANDAS episodes? A. Yes. Children with PANDAS often experience one or more of the following symptoms in conjunction with their OCD and/or tics: 1.. ADHD symptoms (hyperactivity, inattention, fidgety) 2.. Separation anxiety (Child is "clingy" and has difficulty separating from his/her caregivers. For example, the child may not want to be in a different room in the house from his/her parents.) 3.. Mood changes (irritability, sadness, emotional lability) 4.. Sleep disturbance 5.. Night- time bed wetting and/or day- time urinary frequency 6.. Fine/gross motor changes (e.g. changes in handwriting) 7.. Joint pains here is another list from another siteRed flagsThe biggest difference between regular OCD and PANDAS OCD is the sudden onset. OCD usually develops slowly over weeks, months or even years; however, PANDAS OCD seems to come out of the blue with parents saying one day their child woke up a different kid. New handwriting problems, loss of math skills, sensory sensitivities Q. My child has had strep. throat before, and he has tics and/or OCD. Does that mean he has PANDAS? A. No. Many children have OCD and/or tics, and almost all school aged children get strep. throat at some point in their lives. In fact, the average grade-school student will have 2 - 3 strep. throat infections each year. PANDAS is considered when there is a very close relationship between the abrupt onset or worsening or OCD and/or tics, and a preceding strep. infection. If strep. is found in conjunction with two or three episodes of OCD/tics, then it may be that the child has PANDAS What to do if you suspect your child has PANDASa.. Contact your pediatrician. Advocate for your child by being clear about what you want your doctor to do. In some cases, you may have to find a sympathetic doctor who is willing to learn about PANDAS. The PANDAS Clinic at NIMH may be willing to consult with doctors. b.. Have your pediatrician do a throat culture of group A B- hemolytic strep. Ask that the specimen be cultured, not just the rapid test. c.. Your doctor may also do a blood test to assess elevated streptococcal antibodies (Anti-DNAse B and Antistreptolysin titers (ASO)). d.. You and/or your doctor can get information and treatment guidelines from the National Institute of Mental Health (www.nih.gov); type in PANDAS to access the appropriate link. e.. Contact the OC Foundation to help locate an expert in OCD in your area. f.. Have your child retested if there is a resurgence in OCD symptoms. yes my son and best friend both have PANDAS< my son had scarlet fever and strep throat and deteriorated what felt like over night almost 2 years ago, which we have then seen a pattern with 3 other episodes, which gives him the dx of PANDAS, he gets very OCD over letter and turns extreamly hyperlexic, his hand writting goes, he has melt downs over nothings, lots of tantrums, gets mean and will hurt his sisters for no reason ect. my aspie friend well get really depressed, and emotional, have melt downs, becomes unfunctionable, her immune system crashes and she will over night new allergies to the place she can bearly eat. (she is an adult and it looks a bit different) she was previously dxed with tourettes which I pushed her to get tested as I saw it come and go like my son with his PANDAS flares and sure enough she tested possitive, so now with treatment OLE (olive leaf extract) she is tic free as well. she has the history of tons of strep infection as a child and being on spectrum as well but wasn't caught till this last year. so her peices are coming together now. "anti-brain antibodies" These are antibodies present in cases of PANDAS. My son's were 200-400% elevated and no antibiotics or OLE did anything to help. After 4 months of treating this with antibiotics and OLE my son's numbers were worse. We finally started IVIG. My son will get his 6th IVIG next. We've seen a slow but steady reduction of all the negatives we saw come with PANDAS. We've also seen some nice gains. Last year, before PANDAS, my son was on the brink of something big, so close. But we lost him again to PANDAS. With the changes we've seen lately I can tell he's on the brink of a major change again. We started noticing the symptoms right about the time he was fascinated with alphabets and numbers. He taught himself to read at 2. This continued till he was 4. Then he got fascinated by buttons/switches. He would switch on and switch off button forever. That went away a month into OLE. Now they are back. He's now into numbers. He's reciting his multiplication tables as he goes up and down the slide. He's playing hide and seek with numbers etc. The OCD's have changed over the years since the onset of OCD's. The buttons was the worst of them. He broke a lot of electronics in our house and others. He used to go after fire alarms in the hallways.He has stoped the escalators in manhattan subway. It was pretty intense. That is when I started OLE. I continue to give him OLE 500 mg in morning and 500 mg at night. I was giving him therabioc with strep lines in it which I stopped after reading your email. I have stopped it since past 1 month. I still don't see any lessening of symptoms. Besides the obivious symptoms he has severe food allergies, developing new ones everyday. I have no choice but to give him the allergic food. He's also always complained of neck hurting. He complains soon after eating specific foods. I stop giving him those foods. I used to think he's allergic to them. Do you have any advices for me. My son is nearly six and is 40 lbs. I have asked my DAN and he's not given me any specific suggestion. I have started OLE on my own and have informed him about it. He is high functioning at home but outside at school he does not talk at all. He has a lot of anxieties. Even in other people's house he refuses to talk to me also. Our immunologist (who thank God has a lot of knowledge about Autism and PANDAS from what I can tell) said there is no definitive test for PANDAS. He said the ASO and Anti DNase titers just tells you that he has/had strep but doesn't confirm PANDAS. Is that the case? He is very open so if that isn't correct I would love to know more. Guest guest Posted September 13, 2008 Report Share Posted September 13, 2008 We didn't have a PANDAS diagnosis officially but my son had a lot of the symptoms. His Dnase was 960; ASO normal. We did a course of penicillin that a DAN doctor prescribed. His symptoms improved around 2 weeks after we finished the course. I also gave him adrenal 180, an adrenal cortex supplement, as OCD can sometimes be due to adrenal weakness. Another DAN doctor recommended Zithromax if the penicillin didn't work, as Zithro gets the gram negative bacs like strep. The dosage is two capsules once a week which is nice, as you could still keep up probiotic levels. We have that rx on standby in case. My son definitely reacted horribly to anything with strep strains in it, including threelac. VSL#3 apparently does have one strep strain in it, so I have avoided that one too. Interestingly, many dairy products are cultured using strep thermophilus, and my son did get quite irritable and annoying after eating yogurt and cheese but that could also be food intolerance. You would have to run a complete immune panel including titers and hope to find something just wrong enough there to warrant coverage. Kerrie Ok, thanks!
