How do I mix a 28% Sodium Chlorite solution in a 4oz bottle ???

I need to make just one 4 oz bottle of mms, I have the scale to measure it out

in grams etc but cannot remember the ratio (how many grams of sodium chlorite

and how much water to make one 4oz bottle of 28% ) even though I bought the

ebook explaining how to make it I cannot find where I put it. Anyone have the

proper ratio etc ?. Thanks
