Workers Comp and No-fault

I am in upstate NY, planning to start my IMP Oct. 1st. How do

Worker's Comp and No-fault work? I understand there are lots of

different carriers? Do you have to sign up with them in advance? Any

words of wisdom about dealing with them, or recommendations against

dealing with them? ---Sharlene---

I am also in CNY (Syracuse). I do NO Comp or NoFault.

None. Ever. I am looking into doing that as a cash service,

Maybe to ? I think it can be done outside the usual carrier

contract, since it has nothing to do with the carrier. If the

patient wants to see their own doc for Comp/NF, they may

be willing to pay cash.

I made the mistake of accepting Workman’s

Comp and Motor Vehicle insurances (we don’t have No-Fault Insurance here).

Now I have multiple claims that haven’t been paid THREE YEARS after the

date of service. What a complete waste of time! The forms and

delays and lawyers involved soak up hours of time. People will go to two

or three lawyers, a few chiropractors, and everybody wants another set of

notes, copies of statements, more xrays, etc. Remember the lawyers are

the ones with the big TV and telephone ads.

You get caught up in the cycle of everyone

wanting to sue everybody else, nobody wants to settle until they get as much as

they can get out of someone, the chiropractors and lawyers working together to

increase the injury award to improve their own cuts, and having to give

depositions about the extent of injury. I now have a sign up front

stating that patients need to go ahead and pay their bill and then they can

keep any awards/lawsuits they eventually receive. I will see and treat

and document and give depositions but now I get paid, in full, on the

spot.

I can accept delayed gratification as

well as the next guy but I need money to pay my bills.

Sharlene - Yes, I send them to Urgent Care, ER or to an appropriate specialist. I do

not like doing that, but otherwise the work is usually done for free. Delayed

payments,. lawyers requests for records. I used to do Comp/NF but never

got reimbursed. Authorization for medications, imaging, it's a nightmare. Then

with the Comp/NF cases come....DISABILITY!!! The forms, the paperwork, the whining

"I can't wooooork!!!", I can't do all that and remain sane and paid.

My patient's all know ahead of time I do not do COMP/NF. If they don't like that,

then they will go somewhere else. But the work involved for an IMP is

prohibitive.

Thanks for your insights, Tonya. If my own patient gets hurt at work

or in an MVA, can I require payment up-front and then give them a HCFA

1500 to submit to their carrier themselves for reimbursement? How do

other people handle this? ---Sharlene

I believe you can for auto accidents, but not for workers comp. For workers comp, you can only have them see a workers comp dr or bite the bullet.

I have a good relationship with 1 company in area for WC; the carrier pays, but the state sets the fee.

Maybe it helps that I'm not a purist IMP, using 1.5 staff............they DO keep me sane, but it has taken alot longer to get to break even (3.5 years DONE).
