Vaccination consent from and partial vaccination risk form

Greetings,

after my recent wrong-vaccination scare, I have wholly recovered, and

my patients still like me (see my previous post)

It did trigger some better awareness, though, when it comes to

vaccinations.

Two questions:

1. Can someone send me a vaccination administration consent form to my

email address (attachments to the list always get lost) or upload it to

the 'files section' ?

2. Can someone send me a consent form for parents that want only

partial vaccinations? I want my patients' parents to acknowledge the

risk so that I cannot get sued when the kid actually does get polio or

tetanus etc.

I just came across this post, and I'm interested in these consent

forms as well. I'm about to start administering vaccines in my new

IMP, and quite honestly I hadn't even thought of consent forms. I

remember the staff at my old office filling out forms before they

gave a flu shot, and I know I need to hand out a VIS for each peds

shot. What other paperwork/consent needs to happen? what about for

adult vaccines (ie adacel)? any help on this would be appreciated!
