Jay Parkinson invented high tech solo practice like Al Gore invented the Internet. Not quite. He just had better marketing skills and contacts.

Other than that, he's on target about the need for a direct reimburesement model for primary care and the comprehensive tech support and communication tools needed to put that in place.

Kibbe MD, former IT head honcho at AAFP is working with this start up, Myca aka. Hello Health.

Maybe the IMP movement should join forces with these guys, cut a deal to get the software for free for life and give Myca and Hello Health a national network. My understanding is that Hello Health would probably want a franchise fee of some kind in return. The key would be to negotiate that down with the strength of a larger group.

In Illinois, this type of prepaid plan might not be kosher with the state insurance commission who would probably view it as a type of insurance.

Jay gets $100-250 for an office call. That would be nice, but I'm not sure it's realistic. The Minute Clinic gets $59 and is already national. I'd bet on the Minute Clinic. I'm not sure this is a game changer, but expanding the subscription model and easing the regulatory environment for it would be a good thing.

I definatley think this is going somewhere. They are ready to open

another office in January in the West Village. Call me crazy, but I

have some pts who are self pay and they are willing to pay me $80 for

a visit. $100 is not that much more...and if it includes your Rx meds?

People in NY must have lots more money, or much less insurance influence. I get $30-$50 per office visit from insurance ALOT of the time. $100-250 is beyond imagination. Here, i charge $120 and get $40 from ins, $20 from the patient...for an hour visit and 5-10 free follow up phone calls and letters. Fierce competition here would keep Hello Health from making any impact. Why would anyone pay $100 or more, when they can get a house call for $10-20 out of pocket?

I think both are true. There are a lot of people in New York, and some of them have more money than they know what to do with. Some of them have no insurance, or have high deductible plans, or just would rather spend their money on someone coming to them. I think it will fly with a certain segment. It's a great idea. I object for because there are other factors about Mr Parkinson that are particularly annoying that I have discussed before.Ben's ideas about Myca/Hello Health and a national franchise are interesting. I am not sure I would want to get involved in another big organization, but a unifying software (theirs is sure pretty) would be nice.Lonna

Many docs here are drawing the line and just kicking patients out of the office to come back for followup. There is a high turnover of patients here. Either they cant get an appointment (had a call yesterday, her doc said they couldnt see her till April . Something fishy there.) or they are rushed out and "dont get to ask my questtions." We also still have some compettition between insurance companies. Not enough, but some.
