New CPT Codes for Anticoagulant Management 99363 Anticoagulation management for an outpatient taking warfarin, physician review and interpretation of International Normalized Ratio (INR) testing, patient instructions, dosage adjustment (as needed) and ordering of additional tests; initial 90 days of therapy (must include a minimum of 8 INR measurements) 99364 Each subsequent 90 days of therapy (must include a minimum of 3 INR measurements) For outpatient services only. Not to be used in conjunction with codes for physician review, interpretation and patient management of home INR testing for patients with mechanical heart valves(s). Any period less than 60 continuous outpatient days is not reported. ==================================== I guess one submits these codes at the end of 90 days and then every 90 days after that? Would be nice if it was submitted with each INR like a visit code. I know some of you advocate just having the patient seen for a nurse visit and charge a copay for each draw. Or doing the INR with a machine at the time of service -- definitely best medicine, but involves outlay of money initially - possibly getting it back later.
