However, all your nurses need to do is to send a letter to the local MD stating this is needed for transplant approval and can they coordinate the care. If so, there should be something somewhere, or the patient should not be listed (if they are then the center is out of compliance with their own Policies, not good when CMS inspects). If not, then the PCP’s office should be queried. I am under the impression that most transplant centers have that basic screening as part of their listing requirements. We do not believe that you can “require” the patient to get the testing and pay for it. You can have the PCP utilize their own referral system but you should require the bills to come to the transplant center. Basically, the problem I am having right now is with someone who is already listed and has been for several years. I sent updated clinical to her insurance recently to get the auth recertified for another year, and they called me back wanting to see a recent mammo/pap. The last one we have was done a while ago, and per call to PCP they state that they do not have record of any more recent. So naturally one would think that we would just ask the PCP to coordinate this for the patient & let us know when it is scheduled so we can track down the report… I swear, I am speaking another language or something… I am waiting on a response back about what our protocol is… since Bill, I love your email—as always, thanks for the great tip. I’m sure the mammo/pap is a part of our protocol and I can’t wait to confirm this and then remind our administrator about being compliant with our own protocols… though I’m not sure if the continued acceptance criteria will be different that initial listing criteria… I’ll have to see. Ask the coordinator if he/she wants the patient to be transplanted. Tell her that the insurance company can and will deny if the request is not met and that you are only doing what is in the best interest of the patient. Mamo the same. I verify if the insurance will pay full charges, if not we schedule thru our center and pay with KA since is is required for listing. I also informed the coordinators that the insurance company is like a bank and the authorization is like a check. I have not had any problems since.....
