It is

part of our protocol also.

What we

find is that our physicians want to do everything here so we have to get the

auths. However, all your nurses need to do is to send a letter to the local MD

stating this is needed for transplant approval and can they coordinate the

care.

pap” for a patient who I am trying to get authorized for liver txp

(either initially or to recertify txp auth for continued listing), all I get is

flak…

via email from one of my nurses:

“Why do they keep asking US for this when it is her PCP

office’s job??”

Do the other centers out there

require these tests be on record in the patient’s chart if the woman is

over 50???!!

My goodness! I would think

that we want to know this stuff!!!!!

I’m just so frustrated

& I want to know if this is a part of other centers’ protocol & how

you handle things like this…

(something that I am good at). Have you asked the nurse coordinators if

the acceptance criteria/evaluation protocol includes the requirement for a “recent”

mam/pap. If so, there should be something somewhere, or the patient should

not be listed (if they are then the center is out of compliance with their own

Policies, not good when CMS inspects). If not, then the PCP’s

office should be queried.

I am under the impression that most

transplant centers have that basic screening as part of their listing

requirements.

Hi,

Please be careful with this. If it is

a part of the protocol and the patient does not have it. We do not

believe that you can “require” the patient to get the testing and

pay for it. You can have the PCP utilize their own referral system but

you should require the bills to come to the transplant center.

authorization for listing if that Case Manager does not received those

information. The case manager will not be calling the PCP for this information.

protocol.

so quickly!

Basically, the

problem I am having right now is with someone who is already listed and has

been for several years. I sent updated clinical to her insurance recently

to get the auth recertified for another year, and they called me back wanting

to see a recent mammo/pap. The last one we have was done a while ago, and

per call to PCP they state that they do not have record of any more recent. So

naturally one would think that we would just ask the PCP to coordinate this for

the patient & let us know when it is scheduled so we can track down the

report…

I swear, I am

speaking another language or something…

I am waiting on a

response back about what our protocol is… since Bill, I love your email—as

always, thanks for the great tip. I’m sure the mammo/pap is a

part of our protocol and I can’t wait to confirm this and then remind our

administrator about being compliant with our own protocols… though I’m

not sure if the continued acceptance criteria will be different that initial

listing criteria… I’ll have to see.

I just am sick of the

Have you asked the nurse coordinators if the acceptance

criteria/evaluation protocol includes the requirement for a

“recent” mam/pap. If so, there should be something somewhere,

or the patient should not be listed (if they are then the center is out of

compliance with their own Policies, not good when CMS inspects). If not,

then the PCP’s office should be queried.

I am under the impression that most transplant centers have that

basic screening as part of their listing requirements.

Travel safely. When you next ask the

question and you get flak, which you will. Ask the coordinator if he/she

wants the patient to be transplanted. Tell her that the insurance company

can and will deny if the request is not met and that you are only doing what is

in the best interest of the patient.

Many of our patients do not get listed because they have not provided the results for these tests. Many of our patients do not get listed because they have not provided the results for these tests. Yes, we require the same at our Kidney Center.

Medicare will pay for one pap every two years. I pay for the other thru KA.

Mamo the same. I verify if the insurance will pay full charges, if not we

schedule thru our center and pay with KA since is is required for listing.

Meredith when I first became a TFC years ago I only got this once or twice and my response was, " Would you prefer to transplant someone with cervical or breast cancer?" I also informed the coordinators that the insurance company is like a bank and the authorization is like a check. I have not had any problems since.....
