Notification to the Network

We recently had three transplanted patients who had been on dialysis

prior to tx who were not showing on our annual report from our Network.

Does anyone typically verify that this notification was done post

transplant? If so how, just by calling?

If so how, just by calling?[Aguiar, J.] It's up to the tx ctr to notify the ESRD Network a tx has happened. 1 copy of the 2728 goes to the Network (I always sent them w/our monthly activity report), 1 goes to the pts local Social Security office, & 1 copy stays in the record. There's also a death notification form that has to be completed & sent to the Network for any post-tx pts who die.
