Does anyone typically verify that this notification was done post transplant? If so how, just by calling? Thanks! If so how, just by calling?[Aguiar, J.] It's up to the tx ctr to notify the ESRD Network a tx has happened. 1 copy of the 2728 goes to the Network (I always sent them w/our monthly activity report), 1 goes to the pts local Social Security office, & 1 copy stays in the record. There's also a death notification form that has to be completed & sent to the Network for any post-tx pts who die. J. Aguiar Mayo Clinic Arizona ..
