Transplant Identification Cards

Hello all,

I am wondering if I can get your input. We are trying to design

identification cards for our Transplant Patients that can be presented

for all transplant related services, and especially for admission to the

hospital for the actual transplant. I have received one example from one

of you, and would like to know if anyone else does this and if so can

you send me an example of it? We originally needed it to tell our

registration people how to correctly bill and set up the transplant

account, but they are wanting to add clinical info to the card also. So

if anyone has an example or any input on this can you let me know? I

appreciate any help and insight you can give me.
