Anyone know about an " AVDRA " scale for Alzheimer's dementia

(331.0)?

PBM is looking for a moderate or severe rating on this

particular scale to qualify for Namenda and I can't find this on the net.

(They also want a 19 or lower on MMSE by the way.)
