I am particularly interested in whether you are hosting or having someone else host it. I am leaning toward the ASP model, but the fact that I never own the program bothers me. The ASP model does give me a lower cost to start but over time will cost more. Also, I do not have to worry about servers, back up and IT cost with the ASP model. I am pretty good with computers, but I have never done anything like this so I am looking for other people's experiences. Thanks in advance for any input. Guest guest Posted December 8, 2008 Report Share Posted December 8, 2008 if you do asp you cannot use it when you a re on a housecall or like maybe updating templates or other chores on an airplane /traveling; say go sit in t he hotel lobby open the laptop answer your phone calls see what meds they a re on or check for allergies etc A laptop +an emr +a cellphone allow you to give really seamless care while sneaking off to a conference. stuff like that makes me have it not on asp I use Welford chart notes becasue if I type a letter " your mammoragm is normal " it reads that and puts mammogram nl 12-8-08 into the lab section, it read or parses alot of stuff which I think is way cool. AMazing charts is very nice though I must admit I have been playing with it You cannot make macros with it thoguh you can make templates, but you cannot say type ecz and have it spell eczema for you( or metrorraghia..... or other hard to spell hard to type words.YOu have ot get shortkeys for that with AC And alot of people like e MDs People seem to not have found E-CW easy to work withPRaxis is very interesting also I hear.etc.Jean I was wondering what most people are doing for their EMR? Basically you start up a Windows 2003 server in the cloud ie. hosted on Amazon's Elastic Computing platform. You then run Synapse and store your data encrypted in S3 ( Amazon's simple storage ). This gives you the benefit of access anywhere you have internet eg. airport lounges, housecalls, hotel lobby using a wireless G3 card or phone. And you have all the distributed backup, and Amazon's 99.95% uptime commitment behind it. There are no hardware costs .. just the monthly charges. Amazon would charge a minimum of $70/month .. but you can reduce this by turning off the server at night ie. they charge per computing hour. So, you could be looking at " hardware " rental of $35 /month. Now do you require PhD in comsci to set this up? ASP takes the server out of the building, but it isn't the same as running software written to exist in the cloud. The eCW ASP program is a remotely hosted version of traditional software. This means you still need dedicated ways to communicate with the server that are fundamentally different than simply accessing your program via the web from any computer. With eCW for example, you need VPIN routers, particularly software platforms (Windows), etc.- and they charge you a fortune for mandatory start-up training. I asked about eCW when I was thinking about it a few months ago, and someone here rightly pointed out that an EMR that requires 3-4 days of dedicated training to use probably isn't very user friendly. PracticeFusion is Free! ($100 a month if you don't want ads at the bottom of the page - they really aren't that bad.) They just launched a documents management component and are working on lab integration. It's a new company, but the program is very intuitive - admittedly many fewer bells and whistles than eMDs or eCW for example, but a lot of those whistles are things people don't use much anyway. And did I mention it was free? It's nice to plug a $0 into the EMR line on my P & L. If I want to change to a different system due to their high fees or just want another system with better fit, would they let me have my pt record on the PF for free and how (in what format)? My understanding is they have a claims service to handle collections - something like 6% of actual dollars collected which is reasonably competitive with other outsourcing options. Don't think that there's an integrated PM system per se if you're handling claims in-house. Again - not my area of interest.No ePrescribing yet but it's coming. They've launched an in-practice communication tool (which is nice and better than email for communications within a practice). Of course that's not relevant if you're practicing solo. Other additions over the past couple of months include document mgmt and automated patient notification. The point is it is clearly not a complete solution as it exists right now, but they are growing incredibly fast given that they've disrupted the notion that docs should pay for EMRs. They are making a lot of money by pooling data from a growing provider base and putting that money into significant upgrades in the system. In 10 years, whether it's PracticeFusion or another company, the days of docs paying thousands of dollars for EMR/PMS software will be gone. That model doesn't work as it trys to extract profits from the wrong source. The lack of a patient Portal is an obvious hole in the system. Good news is that the entire concept is built upon asking docs what pieces need to be added most urgently. Given that it's a SAAS system, upgrades happen seamlessly and at a much great pace than is possible with traditional software - even if it's hosted remotely, The mechanics of programming are just different for non web-native programs. Email to patients within the system is sure to come soon - but given that they have document mgmt it's not that big of a hassle to transfer messages into the EMR from any outside email address. Of course, every practice is different so feature importance varies for everyone. I don't really care about a patient portal as long as my patients can get to me easily - but that's just me. Similarly, ePrescribing has it's own warts. Yet to be seen how it goes. Haven't found many others using this--haven't been clear why not. So we're going out on a limb a bit after researching for a LONG time--finally decided to just pick and go.
