Would also appreciate a billing address for future reference if she is seen here again in the future for any complications, and I would be curious to know what private insurance she has, if any, in case she comes for services NOT related to her donation…. I have no clue what spooked this girl enough to not want to give us her real name & DOB. She also refused to provide her SSN (Though I can imagine why-since it wouldn’t match her name/DOB!) Thanks! My understanding

under HIPAA law, the information is not being used for anything other than for

payment or treatment purposes.

The patient signed a

release form at point of admission anyway, so we have her permission anyway—

I perhaps should have

mentioned that the first time.
