I have a new question for the group. Periodically the director of our HLA Lab feels it necessary to tissue type a spouse or a child to clarify a transplant candidate's antibodies for listing in UNOS. Who pays for this? Since these people are not really donors are they allowed to be charged to the KACC? Do they need their own coverage for this testing, or is the Center responsible for these charges? Any help would be appreciated. Part A should be available at no cost, and they wouldn't have the Part B

premium.

My thought is: who has the crystal ball that can predict what will

happen in the future not just for this patient but also to the rules you

mentioned about your state's health plan? Also, what if this patient

moves out of state...they will want to have a Medicare covered transplant (even

if it is the 3rd insurance), so they can access Part B for immunos down

the road. Do the rules change at all once this person reaches retirement

age.

I feel better to take the steps to provide the patient with the

most options for the future as well.

was filled out as NOT applying for Medicare benefits when she was started over

a year ago. She was told, why pay $95 per month when they are not going to pay

for anything. Now she has been transplanted and I am concerned about what to

do. I have advised her that this can be risky business, not to sign up for

Medicare, even though it may not pay for anything now

(and even cost her). But I'm not 100% certain that I am right about this. She

is in her late 50's and plans on working until at least age 62 if she can. Our

HI,

The cost is allowable in the KACC. It is

testing that concerns the accuracy of the potential recipients typing, the

typing that is utilized to receive an offer.

And they pay TAP facilities waaaaaay less than the URN ctrs, Julius. At my previous program we were actually approved for TAP, but walked away because the payment they proposed was way too low for us to live with.

The TAP network came about because too many employers were complaining about their employees having to travel & by-pass perfectly good tx ctrs in their own backyards for service. TAP was their answer.

COE= Center of Excellence.

This means the program meets the clinical / management criteria set by URN.

Benefits are highest at this level, & are often enchanced w/addt’l

T & L, no OOP to the pt., etc. On site clinical visits/”inspections”

occur before a facility is determined to meet these criteria. Then the

financial agreements are made & they are eval to post transplant w/various

phases.

TAP= Transplant Access Program. This means

the facility program does not currently meet the clinical/mgmt critieria set

forth by URN. It is phase 3 only (transplant admit only), & does not give

the pt.the enhanced benefits.

Your Mg’d Care Dept should have the

particulars of your specific agreement with URN;.

Hi Julius, we moved from TAP to COE a couple of years ago and it made a huge difference in the #s of referrals we could accept.
