this report from The Center for Medicare Advocacy is a overview of how PFFS plans really work - and in it they make a distinction between "direct-contracting" providers and "deemed" providers so now we know Fred is not making this stuff up ;-) and I'm guessing that unless our programs seek out a contract w/the PFFS plan, we would be considered "deemed" providers and be forced to accept the DRG as payment in full? Please help me with this question.

We have a kidney patient who is covered as

a dependent under his wife’s employer group health plan and has Medicare.

He started dialysis on 07/03/06. On May1, 2008, the patient enroll to a

Medicare Advantage program. He has not completed his 30 month COB, but since he

enrolled to a Medicare Advantage Program, the employer group health plan is

claiming that his Medicare Advantage is now primary and the GEHP is secondary.

I spoke with the Case Manager of the GEHP, and she insists that they are

secondary? Does the rule change when patient enrolled to a Medicare Advantage

Program?

That was my thought as well as I read through some of the

previous

emails. When I call for benefits we are advised the out of pocket

amounts and that the follow Medicare guidelines. Our center has not

discussed this coverage in datail, but reading through these emails it

appears we need to do that! We just have not had a transplant yet

through one of these payers.

How would that be, Fred. When I verify I am told they follow all

Medicare guideline. And when the payments came in they were the

Medicare DRG. Why would they pay extra for the organ ? Do you have

a

regulation that states that ?

That figure is based on immunosuppressive meds only and came from our pharmacy using Medicare EOB's for three immunosuppressive meds. Valcyte is a D drug. This was done for a kidney pt with Medicare as his only source of payment by insurance.
