They should pay primary once Medicare exhausts… usually you need to call to precert though… depends on who the payer is. Guest guest Posted September 17, 2008 Report Share Posted September 17, 2008 When Medicare in-patient days are exhausted the facility can split the billing into in-patient and out-patient charges. Any charges that would ordinarily be billed to Medicare Part B can be billed as out-patient services and will be paid by the Part B plan. Your Patient Accounts department should be handling this. Guest guest Posted September 19, 2008 Report Share Posted September 19, 2008 We will try to get Medicaid witha spend down for the patient: We are asking the patient to collect bills($1,024 Medicare Part B deductible for example) and take them to her Medicaidcase worker hoping she'll qualify for a spend down. According to Medicare, " a Medicare beneficiary is entitled to an unlimitednumber of benefit periods, each of which includes 90 days of covered inpatienthospital services. A benefit period, or " spell of illness, " begins on the first day the beneficiary is furnished inpatient hospitalservices, inpatient critical access hospital services or long term careservices. The benefit period ends with the close of the first period of60 consecutive days thereafter on each of which he/she is neither an inpatientof a hospital or a critical access hospital nor an inpatient of a skillednursing facility.
