Pt. is 69

working full-time and has Aetna.

His benefits are the following:

Calendar

Deductible: $250

MOOP: $1,000

Admission

Co-pay: $500 per admit

Lifetime

Max: $1 million

During my

initial evaluation I found out Pt. also has Medicare Part A. What will he be responsible for? This

Hi Amy,

Lori is correct, however there would be several questions I would ask this patient, mostly due to his age.

How much longer will he be working since he is 69 y.o. That is the key here.

If he stops working right now, will Aetna stay in place as a medigap policy?

If that is the case then he would definitely need part B because Medicare would switch to primary

I think there is a rule with a person who reaches 70, even if he is still working, that Medicare becomes prime? Anyone aware of that?

In the above scenarios, I agree with in encouraging him to apply for part B. That way he has all his ducts in a row in time for transplant.

My opinion.

Amy,

Part A will pick up

hospital deduct/copay/coinsurance balances AFTER the $1024 deductible…

So no matter what,

the patient will always be responsible for the Medicare part A deductible.

Amy,

I would encourage this man to sign up for Medicare part B this next Jan, Feb,

or March, so that he can have Part B as of July '09. Even if he protests:

" I'm still working, I don't need it.. " He should have that ready to

go. Even in his current situation, even if he weren't in need of a possible

liver txp...having Part B would have covered his MOOP.

She needs to speak w/someone who can advise her on how to protect that $$ & not have it subject to asset inclusion. She may need to establish some kind of trust, that she is the beneficiary of, in order to maintain her eligiblity for Medicaid & not have to forfeit the money or just blow it in one month. She should consult w/someone familiar w/estate planning, for example, or even the state's office of elder affairs may have someone who can assist & advise her w/o charging too much. Also, have her contact the life ins company, assuming she hasn't cashed the check yet, & see if they can delay payment until she has something set up & then re-issue the funds directly into the trust, for example, then it won't be coming to 'her' directly & subject to being counted.

Something additional that comes to mind,

is that she can set up her own Organ Transplant Assistance Fund with all,

or some of the funds by capturing a tax I.D. number and a 501 3 © status.

She could set up guidelines in the 501 application that will allow her

to use the funds for travel, meals and lodging to transplant center, assist

with utility bills, rent etc while away for her transplant etc ... just

a thought

I would urge your patient to contact us at

National Transplant Assistance Fund, a 501©3 that allows patients to raise

funds in a way where it is not income for Medicaid purposes. This is an easy

way to raise funds without having to set up of find a separate 501c3. We

provide that status.

Donna,

I know it is frustrating, but you have to be patient and go through the testing. If your GP treats you for the wrong thing it could just make things worse. Do anything you can to relax. A bottle of wine, bubble bath, a good book..... Give the system a chance to work. Big hugs. Madeline http://health.groups.yahoo.com/group/Lungs_on_Vacation/
