Guest guest Posted September 10, 2008 Report Share Posted September 10, 2008 I have a patient with active commercial insurance who took only ESRD Medicare A (on dialysis) and now is trying to activate the B (had a transplant today). Social Security told the patient that since he declined the B when he took the A, they cannot activate the B without documentation that he had a kidney transplant (today). They gave the patient a new 2728 to complete. ESRD Network has told me I am not to complete a new 2728 but Social Security is insisting they cannot activate the Medicare B without official documentation on a 2728 of the transplant. To settle this case and help this patient (who has huge coinsurance for immunos), Social Security is faxing me his original dialysis 2728 to add the transplant information and will accept that but I have never had to do that before???? What has been your experience in similar scenarios?
