Help with iron test results

Hi,

I got the results back on my iron levels and wondered if you could

please give me you opinions.

Iron,Serum 115 11-130

Iron Bind.Cap.(TIBC) 336 250-450

UIBC 221 150-375

Iron Saturation 34 15-55

Ferritin, Serum 80 10-291

Thanks so much.

Your serum iron is high in the range, and your % Iron saturation is

at mid-point. That would look good to my doc. I wish I had your

numbers! I was still low in both my serum iron and % sat last April,

although my ferritin level had come up a great deal. I am feeling

better so I hope that will be reflected in a higher serum iron and %

sat when I do my next bloodwork soon. I have not been able to get the

storage iron out and into circulation very well, due to lousy thyroid

function for so long.

I am not familiar with a situation of lower ferritin and yet good

iron utilization numbers. Perhaps you are just doing a good job of

getting the iron out into your system. Hopefully others will comment.

Do you take supplements or eat lots of beef and other natural sources

of iron? How are your thyroid tests looking? Sorry, I don't remember

your details.

Hi,

Thanks for responding. I have been eating a lot of beef lately After

reading the symptoms of low ferritin I decided I better get it tested.

I have quite a few. Is the ferritin number very low? I really don't

know much about it. I was diagnosed with adrenal fatigue not to long

ago. I don't know about my thyroid, I suspect I have problems there

and probably have for some time. The doctors always said my TSH was ok

even though I have so many symptoms. Thanks to this board I've learned

that is garbage. I have an appointment with Debbie Winder in a few

weeks.

No, I don't think it's really low, just not at mid-point. My doc has

me targeted for mid-points in the serum iron, % sat and ferritin. I

don't know enough to comment on your ferritin level and what it means.

Yes, you will learn a lot more with a good look at thyroid and other

hormones, DHEA, female hormones. Vit B12. You know from reading other

posts that it's imperative to get the Free T3 and Free T4 measured.

You could also check your Basal Body Temp for about 10 mornings with

a glass thermometer, and get the average (I think my doc throws out

the high and low and averages the rest). This is another indicator to

look at with the rest of the tests. I started at 96.2 and have

climbed to 97.7, and it's pretty well reflected how I've been feeling

better and better. When I had a bad patch due to lack of sleep (from

outside circumstances) it dropped to 97.2.

This comes back to the celiac issue again. Anemia is often one of

the first and only red flags that many people will have. I was

anemic at age 6 (wasn't diagnosed celiac till I was 42 and already

had a long laundry list of related issues include hypothyroid) and

that is often when it will first show up, since you have iron (and B

vitamin) stores at birth that will be depleted around age 6 or 7 if

you have absorption issues due to the intestinal damage from

celiac. I know we've mentioned this before with you (at least I

think it was you) and you said you had been tested, but you need to

make sure they ran all 5 of the tests and beware that for some folks

(especially those that have been sicker longer) we just don't make

high levels of antibodies. The stool test from enterolab

(www.enterolab.com)is more accurate than the blood tests (and has

been tested to be 100% accurate for celiacs - i.e., there are no

false negatives for true celiacs) and it will catch folks that

haven't progressed to the end stage of the disease, but are still

being impacted by the lack of nutrient absorption.

Many doctors won't even test ferritin as they'll just use the MCV

(Mean corpuscular volume) from the CBC and if that is low, assume

you are iron deficient or only run ferritin and other iron specific

tests IF MCV is low. The problem with that logic is that B12

deficiencies result in a high MCV, so if you have both you will have

a normal MCV. This is why folks on this list suggest getting the

ferritin tested - it's been around for quite some time that I've

been aware of.

And, I know I harp on the celiac issue on this list - it's just

statistically, that's the most likely problem that people on this

list will have when they have these sorts of issues - there's about

3 million people that have this disease that just haven't been

diagnosed because the majority of the docs practicing out there were

taught it was a rare disorder when they were in medical school. It's

not - it's now considered one of the most common genetic disorders

and anyone with iron or B vitamin deficiency anemia should be

screened.

I have been seeing Debbie for about a year and like her a lot. You

might want to call the office and ask if you can have blood work done

before your exam. I did that for my exam last week. That way we had

the results at the appointment and I didn't have to call back. Blood

work can be done at CPL and is covered by insurance.

Since you are a new patient, she might want to see you first, but it is

worth asking about. My guess is that she will run a Thyroid panel, CBC,

Complete Metabolic, A1C at the very least. They can either fax the

order to you or to the lab. She will also likely want you to have a

saliva test done to test for adrenals and sex hormones, but you will get

that at the appointment.

Best of luck,

Pat

I haven't been tested for celiac. I don't have symptoms such as these:

http://www.medicinenet.com/celiac_disease/page3.htm

I certainly don't have a weight loss issue. I've gained about 35 lbs in

the past 6 months. I can't say on the D or K deficiencies but I do

bruise easily. I bleed normally though. My balance sucks in general,

always had 2 left feet. The infertility isn't an issue. I still have

very heavy periods and my tubes are tied so I'm not having more babies

:-( My potassium is in mid range. I don't know about the magnesium. My

ferritin is 8 and iron is 45. I've been taking a good B complex for

about 3 months so my B12 is 431. I guess it could be a lot better since

the range they gave was 200-1100.

Should I ask for a celiac test and a full vitamin panel?

Roseanne - I think I forgot to mention that you also need the

antibodies test for Hashi's (Anti TPO ab's). Those of us with

antibodies usually need suppression of our gland activity and a TSH

value (measure of pituitary hormone that stimulates the thyroid gland

to put out hormone) extremely low or suppressed. I believe that

someone asked about suppression last week, and it was discussed for

thyca folks, but is usually needed for Hashi's, too. From what I

understand, if we externally substitute our thyroid gland output of

thyroid hormones, then the antibodies (specific to thyroid tissue)

don't think the gland is active, so they stop trying to destroy it.

And hopefully the a-b's deplete in numbers to a level below the lab

value threshold. For me, the desirable lab value is <35. I started

out around 175 (some folks are in the 1000's so this is highly

individual) and they have been decreasing as my TSH has been

decreasing with more Armour Thyroid replacement hormones. I've been

at 60 Anti TPO ab count for awhile. My TSH was .10 the last time.

I've been feeling a lot better since last fall, so I'm eager to see

if the ab's are down again and also the TSH. I'm guessing they are.

So if a doc has a TSH level below which she/he will not treat (like

not below the bottom of the lab range), many of us could remain under-

medicated and continue to feel bad. As far as the worry about loss of

bone density, I have had bone density tests done over the past 5-6

years, including one done last summer, and my results were very good.

I've always eaten lots of calcium rich foods in my life, and since

I've been with my current doc, I've had calcium supplementation, too.

I think we just need docs who know how to cover all the bases and

balance everything while we are being treated for our complex issues.

Sara, I appreciate the information and will do that. I have been having

frequent anxiety or panic attacks and think it is somewhat hormoral

related.

Thank you Pat. I will look into that. I was diagnosed with adrenal

fatigue or exhaustion not to long ago and there is probably some thyroid

problems and hormones out of whack. I'm glad to hear that you like her.

Do you usually go back fairly quick after the first appointment? I

guess what I really want to know is does she see you and work closely

with you at first until you are on the road to recovery or does she let

you flounder on your on.

Have you taken your basal temps? If

you are hypothyroid, your bone marrow does not get warm enough which causes the

ferritin issues. This is an oversimplification. The book below

explains it very well.

If you are reading, one of the best books

you can read is “hypothyroidism: the Unsuspected Ilness” by Broda

Sorry you are having to go through that. I went through years of it

before being properly diagnosed and treated. Burnout from thyroid being

out of whack and throwing everything off balance. You try to make up

for the energy loss and keep pushing yourself and it often results in

the anxiety disorder. My body is soooo much more peaceful now. You will

get better with proper testing and treatment.

Does anyone know if there is a better form or iron that won't upset the

tummy but will absorb well? The ferrous sulfate she prescribed is 65mg

of elemental. I don't find this high in a chelated form. I can take a

couple or more to get the same dose but I wondered if they were

identical or better than the tried and true crap the DR would prescribe

cuz that's all she knows. Any experience with them?

She will sit down with you and outline a protocol that she wants you to

follow and tell you when she would like for you to come back for a

follow up. Then she leaves it up to you to make the appointment.

A couple of examples, I went to her in May and she suggested that I do

an iodine loading test and get back together after she gets the results

back. I paid for the test kit, but then I went on vacation and sort of

forgot about it. Finally, in November I find the kit, do the test, and

mail it in. In Dec after they got the results, her office called to

schedule a follow up. Now, it was my fault that I didn't follow through

in a timely manner.

I went to the follow-up appt in Jan. We made some adjustments and

eliminated Vytorin. She gave me the lab sheet to have blood drawn and

suggested that it be around March 8. She would like to see me back

after that (or at least schedule a phone consultation). I have to be

delligent in getting the blood work, and call and schedule the appt.

Bring any medical labs and history that you have with you. She will

read through it. Might even be good to put together a chronological

list of symptoms and events to give her.

relationship between these 2 since the doc spoke faster than I could

keep up with.

The weight issue can go either way - I lost about 15 pounds when I

went on the GF diet. It's now a misconception that one must be

underweight to have celiac. Folks sometimes overeat because they're

compensating for the lack of absorption of nutrients. For many

people, you don't have to have overt GI symptoms - I certainly

didn't think I did - I just thought I had a " sensitive " stomach. The

eating lots of meat and still having a ferritin level that low is

one red flag. Also, your folate was really high, which means it's

not likely being properly metabolized with your B12. Have they

measured your homocysteine? That can be high if you are not able to

properly metabolize the B12. My B12 was in normal range, as well,

but my folate was sky high before I went on the GF diet. Here's an

article that talks about " silent " celiac disease due to anemia:

http://www.pubmedcentral.nih.gov/articlerender.fcgi?artid=1276329

And, here's an article that discusses the elevated prolactin being

correlated with celiac:

http://www.celiac.com/articles/766/1/Elevated-Levels-of-Serum-

Prolactin-Linked-to-Celiac-Disease/Page1.html

So, it would definitely be worthwhile for your doctor to run the

celiac panel - just beware that you may be one of those that doesn't

produce high antibodies to anything (you had elevated, but not above

normal thyroid antibodies if I read your previous postings

correctly - that's exactly the way I've been). Here's a summary of

the recommended tests:

- IgA and IgG Anti-gliadin antibodies (AGA)

- Anti tissue transglutaminase antibodies (tTG)

- Antiendomysial antibodies (EMA)

- Total IgA (to rule out false negatives due to IgA deficiency)

- HLA typing (this is the gene test to determine if you have the

genetic predisposition - it's not absolutely necessary, but provides

really useful information)

Yes, didn't we talk about this...? :-) The Solgar Gentle Iron, Iron Bisglycinate, very gentle, non-constipating. It doesn't have to be 65mg. Different form of Iron. I take 2 a day. My Ferritin started out at 29. In four months on just a multi with Iron it went up to 64. Then I started the Gentle Iron, 2 a day. In five months it went up to 99. In five more months it went up to 191.

I guess my lab results didn’t read

that book. J

Re: Help with iron test results

I've been a meataholic and eaten loads of green leafies for over 4

years

on Atkins. I should have iron levels to be proud of. How in the world

could I still be low? I always thought a supplement was created for what

you lacked in your diet. I'm guessing there is an absorbsion issue or

something is leaching it faster than I can eat it.

Ferritin 8 & iron 45. I do think this is higher compared to my iron in

the past though. How long has ferritin testing been around. All the

relationship between these 2 since the doc spoke faster than I could

Yes, but I got concerned about the mgs

being so different. I wasn’t sure if it would do the same thing, just be

easier to take. Dr said to take the 65 over the 45 of the feosol cuz she wanted

me to have the max. I’m glad to know it was so successful for you. I’ll

give that one a try. Thanks J

My labs were pretty darn normal too.

None the less, I was definitely hypothyroid. It was very evident from my

physical exam, symptoms and basal body temps.

Per his book, anything under 97.8 is suspicious for hypothyroidism.

Re: Help with iron test results

Have you taken your basal temps? If you are hypothyroid, your

bone marrow does not get warm enough which causes the ferritin issues.

This is an oversimplification. The book below explains it very well.

If you are reading, one of the best books you can read is

“hypothyroidism: the Unsuspected Ilness” by Broda .

I just hope I can get help from a doctor

soon. I’m going to pray that Dr. Corkran will treat me. I’m just

not sure if I should do the month of iron first, finish the ultrasound from the

gyn to get her opinion on why my uterus is enlarged, etc. or just go ahead and

make the appt. I’m afraid she’ll just tell me to wait and see if

the anemia thing goes away and I’ll be fine. I wish I had the money to go

to Hotze.
