Guest guest Posted December 19, 2008 Report Share Posted December 19, 2008 Hi Robin, Just to double check, the cancer was not discovered until the donor was open and the surgeon could see the kidney, is that correct? And, at that point the transplant was cancelled? Or, was the kidney removed and then the cancer was discovered? The reason that it is an issue is that if the kidney was removed as the beginning of treatment for the cancer then the donor is responsible. If the cancer was discovered after the kidney was removed and then the transplant cancelled, the center is responsible and the organ recovery costs go on the cost report including all physician and surgeon fees. I also hope that during the informed consent process for the donor for evaluation and surgery that the donor understood that if treatment was required, that the donor not the center is responsible. The recipient opted on waiting to see what the findings were before she received anesthesia. After the kidney was removed from donor it took the entire day for pathology to identify that it was cancer. Then the transplant was cancelled. Therefore, ALL CHARGES for DONOR/RECIPIENT (inpatient hospital charges, anethesia and surgery costs) must be paid by the transplant center. Correct, Bill? Guest guest Posted December 19, 2008 Report Share Posted December 19, 2008 Hi Robin, From what you are describing, the recovery surgery was not to be a part of the cancer treatment because there was no diagnosis. The decision to not transplant was only made after pathology finished its testing and made a diagnosis. In this case, the center is responsible for all donor and recipient charges. Yes to your direct question. Had there been a needle biopsy (if that is even done) and the decision was made that there was cancer and then the kidney was removed as a part of treatment for cancer, then the donor would have been responsible for the surgery. Additionally, the recipient’s charges, if the recipient was admitted, would have been the responsibility of the Transplant Center.
