the address provided on the form.

Registration confirmation will be sent by e-mail only, so please print clearly

and legibly on the form.

(you may send the completed form back by e-mail, and mail in your payment)

If you will need a receipt for reimbursement purposes, please indicate that on

the form somewhere and one will be mailed to you. Again, please print clearly
