Guest guest
You don't need any note from a doc or any justification in Texas. Just call the State Health Department in Austin and ask for a vaccination exemption form. They make you request it a certain way. They then send you some official request form, and then you send it to them. They then send you a waiver which you fill out and notarize yourself and take to the schools. If you order via the net, you can do the whole process in about a week. Its easy. Heidi N **************The year's hottest artists on the red carpet at the Grammy Awards. Go to AOL Music. Guest guest
Just go to the TX dept of health website and you'll find the exemption application. I asked for 5 copies for each of my kids and they got them to me ASAP. We are still hoping that the exemption will work for my oldest son who has so far passed through fire and paramedic school w/o the hep B. We'll see if he is allowed to do his paramedic clinicals this spring w/ just a TB test. He's willing to do it if he has to, but he's rather not. Medical pros file the most Hep B vax injury lawsuits world wide. Tina in Hockley
Guest guest
Thank you Heidi for answering me, but this is for schools, I already have exemptions for two of my kids for their schools, I'm now asking about what to do to get vaccinations exemptions for the permenant residence. Does the state department in Austin also give exemptions for individuals seeking permenant residence and do not want to get the vaccinations ? To which way should I go? Thank you so much Fayrouz Re:requesting an individual waiver based on religious or moral convicti You don't need any note from a doc or any justification in Texas.
