Okay, you 've got me stumped on this one. I feel a bit foolish asking.

I've never heard the term " rectal washes " . Is it the same as an enema?

Hi, If our section patients are having difficulty with wind we do 2 things,

1. Get them moving, the more they move the less the problem

2. If its still a problem give them peppermint oil 1 tsp in warm water with

2 cocodamol (dispersible) up to 4 times a day.

I have never heard of using rectal washouts, I used to use them years ago as

pre op to bowel surgery.

Not neccessarily, more like the " unatural path " , like inductions cause their

back hurts...which require interventions that are " chosen " like use of

cervical ripening, all the way to epidurals, instrumented deliveries, etc. I

dont beleive in non-medically indicated intervention...I think it is

dangerous and that most times women are not given a true informed consent

before they jump right in.

I don't mind intervention when it is " needed " , but I really believe in

birth...I trust it and I believe that it takes alot to screw up a nice normal

healthy pregnancy and we often times try our best to do that with uneccessary

intervention! I like to leave nature to its course, labor when your body and

your baby says its time, have patience with the process, provide the support

women need to empower them through birth and be part of the miracle of it all.

I'm not saying I don't do technobirth, on the contrary, I often have the

challenge of caring for a family in a high risk setting that WANTS as little

intervention as possible that unfortunately requires much intervention! But

somehow it seems Ok with me when it is part of the " natural " process for that

particular birth and not a " selfish " ( " my nanny will arrive Wednesday so I

want my induction scheduled for Thursday at 0800 and have anesthesia meet me

at the door for my epidural placement " ) choice.

Yes, it is a " place " that I have arrived at after years of technobirth int he

hospital...I chose to work at a place that trusted birth and provided the

type of care I hoped to provide, and for many years it was so! Yet here too

things are changing and more and more I see people forcing control of birth.

I am tired of watching this progression and would probably be most happy in a

true birthing center....there's talk of one starting here soon and I'll be

there for sure!

Till then, they can give me all the natural childbirthers they want! I love

it! The satisfaction I take away from these births is overwhelming as it is a

true honor to be present and a part of these miraculous births.

your thoughts are mine exactly!!!! Although I've only been doing this

about 2 months, I think I came into OB nursing with strong opinions about

'techno-birth'. But to actually see it in action was eye opening. Its

amazing what some women will let a doctor do them without even asking

questions. And when they do ask questions, the doctors are able to talk

their way into what THEY want to do.

I had a couple last month that I was really looking forward to working

with. They came in with an actual written birth plan. Wanted to walk and

drink during labor. They did not want IV's, continuous EFM, pain killers,

internal scalp electrodes or IUPC, and just about every other intervention

you could come up with. Well, all went well until about 6-7 cm dilation.

She couldn't handle it anymore. I tried to work with her through the

contractions, tried to dissuade her from the epidural, but it wouldn't work.

Of course, as soon as it was in, the decels started. I dont' care how many

people tell me there is no correlation between epidurals and decelarations,

I'll never believe it. She went on to have an ISE, IV, O2, NPO, and probably

every other letter of the alphabet LOL. I see it all the time, but it was

particularly disturbing with this couple because they had so specifically

planned everything out. Once things settled down a little, I strongly

advised them to go with a nurse midwife for their next pregnancy.

I have a question after all this rambling. How do you push for more

natural childbirth for your patients? I'm new at this, so I don't want to

step on anyone's toes at work by chasing off doctors and all of their

interventions just yet. I also deal with a very low income, under-educated

population. They want to come in, strap down, and be put out. But I'd

really like to be a stronger patient advocate.

Thanks for listening, and I'd love to hear more from you on this subject.

I hope that you get your birthing center. I have a friend who's delivered 8

children in one, and she swears by them.

Debbie, WOW, that sounds great! Will you get the full 3000 or will they take

out taxes. I received the same with my last L & D job. It is especially nice

with x-mas right around the corner. Lynn

Debbie, I gave your original post to my NM and she was going to pass it on to

the director of nursing - she was very interested. I am copying the letter

with the breakdown of imbursements on it. Her question was: what state are

you from? I'm not sure why that matters, but she was curious.

I have just been referred a 3 y.o. male patient with chronic eczema. He had cradle cap as an infant and eczema started after introduction of solid foods. He was breastfed exclusively for 8-10 months. First foods were peas, carrots, and potatoes. Added cheese and yogurt at 2 y.o. The distribution is on his abdomen, underarms, cubital and popliteal creases. Very icteric and hot water aggravates the condition. Itching keeps him up at night.He has a had food allergy testing with egg whites and soy IgE positive. He does consume wheat and dairy. He has had one round of oral antibiotics with a skin tag surgery at 1 y.o. As well as 4 day hospitalization for Rotovirus with IV antibiotics for 3 days. Also has had traditional schedule of vaccinations.No Family history of atopic triad. His previous treatment has been Omega stix and Chinese herbs but compliance is low. Has also been prescribed topical cortisone but does not help. Also given Dermasea topical with little relief. I have only had a 15 min consult with his parents who asked about UNDA's.
