Hii Brad - What are the results of her labs? Did you get a free T3 and Free T4. I would recommend switching to the SR T4/T3 blend. If she is taking 175 mcg of Synthroid - then you would want to give about 109 mcg of SR T4 and 27 mcg of T3. This is based purely on the amount of Synthroid that she is taking now, not on lab values of TSH or T3. The dose above would be the T4 equiv. of 217mcg. You want to increase the total T4 by about 25% when switching to a sustained release (due to a decreased absorption in a sustained release formula) Since T3 is 4 X more active than T4, you give 1/4 the dose of T3 --which together make up the total T4 equivalence. Did that make sense? You will want to monitor her FFT about 8 weeks after starting the dose, it takes about that long for the TSH to get to it's steady state. you can test sooner for the free T's if she is symptomatic and then adjust the dose
