From my experience with clients there is condition called atrophic vaginitus, that can occur during pre menopause or even during breastfeeding, where the over-the-counter lubricants just don't help. One pre menopause woman, still in fairly regular cycles, said she tried everything and was relieved to have her Dr. tell her she had an actual condition where the vaginal tissue was thinning and friable, due to lack of estrogen. She commented that she felt she produced an good amount of her own natural lubricant, arousal fluid, but intercourse was still extremely painful.

Dr. Hilgers prescribed Estriol Vaginal Cream to be used on days Peak +3 to Peak +12 of her cycle and it brought great relief. I believe this is a bio-identical hormone preparation so he prefers it to the Premarin Cream that is widely prescribed but derived from horse urine.

It did not alter her mucus observations or her ability to determine sensation as it was a very small amount used at bedtime and only in the post ovulatory time.

Perhaps one of the NFP Medical Consultants or a compound pharmacist can speak to this with greater detail in regards to your client.

I would

suggest specifically the KY “Silk-E” or a silicone-based lubricant

such as Sliquid. They work much better and for longer than traditional

jellies, and usually do not have glycerin, which can promote yeast infections.

Vaginal dryness

I know this has come up before, but

I need some help…..

I have a client who has been

experiencing “extreme vaginal dryness” for the last 3 months.

She also has been diagnosed with diabetes and pre metabolic syndrome- or

maybe with metabolic syndrome.

She has tried KY Jelly and monostat-

both over the counter obviously. Neither has helped.

Her doctor recommended the birth

control pill. She wants to know all her options before she goes back to

the doctor in 2 weeks.

supplementation. That

can be achieved apart from contraceptives. It can be used in cream form,

perhaps she should consult either an NFP only physician, or perhaps a

Naturopath.

Here is a web page on understanding the effects of personal lubricants

on sperm function. If this client wishes to get pregnant, she may be

intersted in this. Quite a few good references listed indicating that

there may be "a deleterious effect of personal lubricants on sperm

function such as motility, including a decreased ability of sperm to

penetrate cervical mucus after exposure to lubricants. Several studies

show that: KY Jelly, Astroglide and Replens killed SPERM EQUIVALENTLY

TO CONTRACEPTIVE JELLIES."

You might have done this already, but things I ask

about:

Undergarments

detergents

softening agents

use of sanitary products

bath soap

diet

drinking enough water

marital status

avoiding, achieving

timing in her cycle (what do her charts look like?)

types of cycle

The key is finding out WHY there is vaginal dryness.

Is it subjective only?

Has she had previous procedures on her cervix?

Taking medications? vitamins?

I think perhaps Mucinex or " Tussin " products could

even help without causing hormonal upset, etc.

I always ask questions. Never outgrew being a 3 y/o I

suppose!
