It almost sounded mean which was not my intention....I probably should have used the word " lifted " from another list that didn't quote a source so that I could not pass that info on. I do have sticky fingers when I see something that looks like it could be helpful. At least let the nurse or doctor take a look at it. I realize it frightening and I dislike dentists myself, however abscess teeth are nothing to fool around with.. Best of Luck! If I can get an oral surgeon to remove it, I'd be covered insurance-wise & avoid the root canal. I'll call my insurance co. tomorrow, but just wondering if anyone out there knows. Thanks! You summed up some of the same thoughts I was trying to state. I have also thought about a cash only business in the past. I have known of a few doctors who have done this. Usually they already had a large patient panel before dropping their contracts, or were known as the local diet pill/narcotic doc. Billing in america is a bizarre duck. In a capitalistic society doctors have let other entities decide how and how much they can bill. Generally based on fear, if I don't accept the terms of the contract from the government or company A, and Doctor B down the street does, then he will get the business and I will not be able to pay my bills. Or if I don't contract with company B,C,D, and E which provides most of the insurance coverage in the area, I will have no patients. Even Gordon's idea of being able to show clinical outcomes to insurance companies in order to get a better reimbursement strikes me as trying to do a better job at home chores so that you can go back to dad and ask for a bigger allowance. (no offense meant to Gordon) The problem with this is that it is just as likely to result in a lower allowance to your siblings instead of an increase for you. Again thank you for your reply. I will look into these discount groups, I hear them advertised on the radio frequently. I would go all cash, but my fear of not enough patients keeps me from doing this, at least at first. I have one employee who is currently taking home more than I am, has better benefits than I do and I am also paying 260/mo in work comp insurance (calif). Does anyone have info that might help me with regards to whether I really need the work comp insurance for only one employee. I inquired with the state but have not heard back yet.......Thank you. I moved from my hometown and away from Chicago to avoid the 30-50,000 rates. The 60-100K would be for both my wife and myself. We are on a 5 year ramp to full malpractice rates. We take a part-time discount for my wife and have asked the same for me this year as I see less than 20 hours of patients per week. This saves 30% on malpractice rate. Some of the doctors of this group might also benefit by asking for part-time until the hours make them full-time. In addition, I have contemplated swwitching to a solo practice. My wife or I would need to resign to stop practicing medicine, but could still likely see patients for the history portion of the exam. This would elimiate the need for staff and save a significant amount in expense. Not optimal, but I always consider my options. Lastly, I am looking at tail insurance, usualy twice the rate of current policy. If the cost cannot be balanced by savings in salaries by efficiency with the EMR, I am prepared to close the doors and start this experiment in a physician friendly state. I am also consdidering a fellowship to advance my training, but I really love general internal medicine. Honestly, I am thanful for the experiences I have lived so far. I diagnosed my twin brother with rhabdomyalysis. It took several phone calls and the stark promise that I would leave my medical rotation and fly 3000see him in the ICU in the morning. He was in the ICU when I walked through the door, but his kidneys didn't shut down and his potassium never bumped over 5.0 despite a CK of 130,000. The "doc-in-the box" in California had told him he couldn't possibly have rhabdomyalysis. Thank God he was started on manitol and lasix immediately. My point is that we are smart enough to do any number of things for a living. If I have to walk away from medicine, I have already healed, saved, and comforted many people. It would be sad to not continue, but I will not live under a bridge in a cardboard box just to practice medicine. If I cannot generate income for my family, internists in Illinois may become more scarce. Since starting college, I have aveaged $26,000 in income per year. This does not include medical education debt repayment.
