Guest guest Posted January 8, 2008 Report Share Posted January 8, 2008 Hi guys, I just had the decompression surgery on 11/14. I am now thinking about applying for short term disability. My neurosurgeon's office is willing to fill out the paper work for me. But, I heard that I need a worksheet that is something like a " functional capacity worksheet " . Also, I am a bit confused as to how I apply. I probably can't apply through work since I was only working part time as a church secretary and I did not receive any kind of benefits through this job. Do I file this straight through the government? I hope I am not being ignorant but I have never applied for anything like this and I am at a loss when it comes to what to do. If anyone could please help me, I would really appreciate it because my husband and everyone is pushing me to get this done and I have no idea the first place to start. Guest guest Posted January 9, 2008 Report Share Posted January 9, 2008 Joy, You would be applying at the Social Security office near you or look up # under US Govt in phone book to call. Having a doc to sign papers for you is big help. The report you refer to is telling SS how you are able or not to make it through a day. What you can do around the house safely/without pain/falling/etc. What others have to do as you cannot. What is a normal days activities for you and how often you have " good days " . Please keep in mind that SS is comprised of a lot of a----holes who can decide that if you can do ANYTHING then you can work. I am not telling anyone to lie or falsify the report BUT carefully read the question and watch what you tell them. Keep a copy before turning in. I was denied my disability in large part to the fact I " admitted " to attending church on occasion/with help!!!Obviously that meant I could punch a time clock???? Do report answers on seperate paper in pencil first. Let others who really know you proof read it first before filling out the actual forms. What we are trying to convey from thoughts to actual written words can get really messed up and get us denied.
