I am thrilled to be able to give you some more great information from Dr. B. As

always, he has been kind enough to take the time to help our list and it's

members. It just dawned on me that perhaps we make too many assumptions and

that some of the newer members may not know who Dr. B is. Thus:

Dr. Paolo A. Bolognese

A native of Torino, Italy, Paolo A. Bolognese, M.D., graduated cum laude from

the Medical School of the University of Turin. In 1990, he completed his

neurosurgical training at the same university under the guidance of Professor

Victor A. Fasano, an international leader in the field of high-tech applied to

neurosurgery.

During this time, Dr. Bolognese became the leading worldwide expert in the field

of laser Doppler flowmetry applied to neurosurgery and the top European figure

in the field of neurosurgical intraoperative ultrasound. Upon the death of his

former mentor, in 1992 he accepted the invitation of Dr. H. Milhorat to

transfer his laser Doppler research to the United States and to be retrained

under Dr. Milhorat at SUNY Health Science Center at Brooklyn.1n addition to his

U.S. neurosurgical training, Dr. Bolognese became the first trainee of the

Fellowship in the Surgical Management of Chiari I Malformation and Related

Disorders under Dr. Milhorat.

In August 2001, Dr. Bolognese joined Dr. Milhorat at the Departments of

Neurosurgery at North Shore University Hospital and Long Island Jewish Medical

Center and as Associate Director of the Chiari Institute.

1.. Do you believe that Chiari and/or SM possess an autoimmunity factor?

CMI is a mesenchimal disorder, as well as many autoimmune disorders;

in CMI pts, rheumatoid factor and ANA titers are frequently moderately elevated;

at the moment we cannot say anything more

1.. Is there any relationship between CM and pineal cysts? How do you

recommend that these be treated?

in our experience, we did not see any higher incidence of pineal cysts in CMI

patients, when compared to the non-CMI population

1.. What do you think about Cold laser therapy?

I used it in Europe in the mid 80's with good results for pain and decubiti;

properyl used, it is another useful tool for CMI patients, but it cannot help

with severe pain

1.. How is cerebellar ptosis diagnosed and treated?

it is easily seen on MRI's;

in our experience, its presence is almost always linkable to Tethered Cord

1.. We often have list members trying to differentiate between potentially low

pressure and high pressure headaches. What do you feel are the most common

symptoms that would help to classify these?

high pressure headaches: whole head or suboccipital pain, described as a

sensation of pressure, often pulsatile, exacerbated by exercise, relieved by

resting; the pt sleeps with the head up

low pressure headaches: the pain is referred to the vertex, frontally, and

behind the eyes; the pt is pale, nauseous, and sickly; standing up makes it

worse, while laying flat makes it better; the pt sleeps in a flat position

Please let me know if you received them. The mail was shown as going through

and I have had other replies. Could the hospital system have picked them up as

spam? Do you know of Dr. M and Dr. Kula received theirs??? I'm so sorry for the
