If pain is localized at nipples, probably still nipple yeast. Strains are resistant to nystantin, it's just candy to them. Use lotrimin topically to nipples after each feed. It is locally effective, not systemic, not absorbed, not digested, OK for baby. Don't wash off after feeding. Use it 2 weeks after pain subsides. Usually pain gets better in 24 hours. IF she has shooting pains through her chest and it's ductal yeast, in addition to hygienic measures, Diflucan 400 mg loading dose day one then 200 mg per day x 28 days (Hale). To treat baby: diflucan suspension. Boil any plastic that touches her breast or baby''s mouth. Treat 2 weeks after pain subsides.
