So my point is, he shouldn't be ruled out of a transplant evaluation, especially if that advice is coming from an internist. Unfortunately these people are money grubbing or else they wouldn't charge 5 dollars a piece for this form. Before Sharon had her brain surgery we filled out one of these for her, and later I filled out one too. They provided hers, but I had to pay 5 dollars for mine. Arrrrrg.
