Hepatic Encephalopathy is one of the most dangerous complication of Decompansated ~Liver condition, which rarely is caught on by the patients simply because there has never been a pill or a procedure that a doctor can prescribe. So most people are completely unaware of the HE progression until one day it becomes painfully obvious that something is so not right with them, that it can not be ignored anymore. And then they go to their doctor and if they are lucky, get a off-label prescription of Lactulose, and thats where the real problem starts. The build up of Ammonia in the digestive tract can be so bad, that most people would have a bowel movement once a week. Ammonia causes a complete change in the gut's flora profile, with pathogenic bacteria rapidly colonizing the are where the friendly bacteria once used to dominate. That causes the bowel movement disruption which in turn causes bile occlusion and by the time any bile gets into the system it is already so stale and bitterly acidic that it burns whatever would have remained of the healthy flora. If it were possible to thwart and keep HE at bay, terminally ill patients could enjoy years upon years of life extension and lead very normal life and be able to function. While there is no " magic " cure for encephalopathy, it's symptoms can be delayed and treated. Encephalopathy can only be reversed if the underlying cause is stopped. In most cases, liver failure. A transplant would be needed to treat chronic liver failure. 70% of individuals with liver failure will experience some form of encephpalopathy. 30% may end up in a hepatic coma. Brain damage from an hepatic coma caused by encephalopathy may not be reversible. The most common treatments are diet, Cathartics (lactulose), and Antibiotics. Other treatments include: L-ornithine L-aspartate (LOLA), Zinc, Sodium benzoate, sodium phenylbutyrate, sodium phenylacetate, L-carnitine.
