It seems that a significant number of mothers who experience engorgement and milk flow while pregnant are on the verge of miscarriage/preterm birth. While I recognize that your post didn't say anything about milk flow, it might be a good idea for the OB to check on the safety of the babies.

PTP- mom is 22 weeks pregnant with second child. Twice over the past few days she's experienced a sudden heavy, engorged, hot feeling in her breasts. Both breasts get hot to touch, and then turn bluish. Sensation is centered around the nipple and areola, and nipples tingle all the time. She will be checked by MD on Monday to make sure nothing's wrong. Assuming everything checks out OK, I'm thinking this might be some variant of vasospasm/Raynaud's (no history of this). But I don't have a ton of experience with breast changes during pregnancy- Does this sound familiar? Any other thoughts? , IBCLC, LLLL

The fact that this is bilateral is interesting.

I once worked with a mother who delivered prematurely at 33 weeks after she had

one of her breasts drained for a seriously infected abscess. She was infected

with Strep A and had been told by a consulting phone nurse that 'her milk must

be coming in early'.

Just watch closely for infection symptoms.

I have talked to the mom and fortunately she had already talked to her MD on the phone about that on Friday. So it wasn't a surprise to her. We had a good talk and she is going to let me know how this progresses.

Not sure. I don't think contractions have been reported in women whose milk seemed to be "coming in" prematurely. Well, at least not at the start. Also, this mama-to-be does not have milk flow, so it could be something entirely different. I honestly don't know how urgent this might be. I just thought she should mention it to her OB. I think I'd probably say something to the effect of "If you seem to start having strong Braxton Hicks contractions, don't wait until Monday to see your doctor. Call him on his emergency line and explain the symptoms you are seeing in your breasts and that at the same time, your Braxton Hicks contractions are getting stronger, and ask if you should head to the ER instead of waiting to see him on Monday." I’m glad she has talked to the OB – it is a myth that preterm labor moms will have contractions, or what feels like contractions. I remember a quote from the mom who started the triplet connection, her triplets were babies 7, 8, & 9 – she had labored & birthed 6 kids before. She went into the hospital for some testing and was shocked to be in labor. She stated preterm labor is insidious, silent & deadly. The symptoms are very different. I had a student call about not ‘feeling right’ who was in preterm labor. She was having a lot of discharge (very watery), a backache (she thought left over from a recent bladder infection) and the baby wasn’t moving much. I told her to call the OB NOW and she was found to be almost 3 cm dialated. Anything that doesn’t feel right should be reported – especially the above symptoms with feeling of pelvic pressure (like the baby is pushing down). I’m glad things are going well with your client.
