I'm paperless now. I have clients fill out intake online (using a secure server thru Formsite) and paste that into a spreadsheet, and also into the Bento app, which I use for charting on my iPad. It has really saved time! Also, I was surprised by how much my clients seem to like it. I've had clients comment on how much easier it is to fill out the forms that way, and many are excited to see the iPad technology, and the Square interface if they pay with a credit card. BTW the Square for iPad is great - you can enter your consult and product fees and then when a client pays, you just tap the things you are charging for, and it can total and calculate sales tax if necessary.

One thing about digital record - I have both a local automatic backup using time capsule and a wireless giant hard drive, and remote backup, using the Carbonite service. Both secure. Gotta have belt and suspenders!

If you have would you mind sharing your template? If no one has I'm happy to share back if you are interested.

Hi Jaye,

I do pay for Formsite, because I use some of the feature (I think I had to pay

in order to link it to paypal, which I do for class registrations. Not for

consult fees though.)

I'm not thrilled with the interface for setting up your form (took longer than I

think it should have) but it has nice results features. I can have all or part

of the results automatically emailed to me (like just her address and contact

info if I want) or I can export a table.

I paste that table into Bento for charting, and Numbers and Excel for

generating reports and financial data. (Because the mail merge feature on

Numbers/Pages is not quite what I need yet, I use Numbers for most things, but

Word for the reports. Serious PITA, but I expect Numbers will figure it out

soon.)
