Turkish-English : Sevkli Kurum

Hello everybody,

I am translating into English a Turkish report titled

" Sevkli Kurumlar Doktor Muayene Bilgi Formu. "

I have translated as " sevkli kurumlar " as " transfer

institutions " but if there exists another, specific

term that should be used, I would appreciate

correction. Or any suggestions.
