We will also be focusing on getting the maximum benefit from each

discipline's input. If the client has trouble learning their medications, we

are encouraging an early OT referral for cognitive strategies. Likewise,

if the client has an order for a home health aide, we would like an OT

eval ASAP to work toward independence. Clients with falls are being

referred to P.T., etc. etc. We hope to learn to really get at the core

issues for each client early on so that they can assume their own care

and/or recover as quickly as possible. This will allow us to balance out

for those clients who need high levels of care over the long term.
