We are required to get the medicare forms signed by referring physician's for

outpatients. The way we handle it , is that the initial referral (which 99% of

the time is Eval & Treat) serves as the initial approval for treatment. We send

the forms, keeping an unsigned copy until the signed one is returned. Our

intermediarry has not had a problem with this. And I am not about to rock the

boat. There would be huge delays in treatmnet if we did anything else.

Faxed forms are not acceptable. They do want an original signature.
