Guest guest Posted April 18, 1999 Report Share Posted April 18, 1999 Steve - When we take an AmeriHealth HMO patient (This is a subsidiary of Liberty Blue Cross) we do the whole outpatient case for $255.00. The break even point (BEP) is at the fifth visit. It would seem that we're assuming financial responsibility for the entire case for that amount, which is a bit like what you described. We had a plaque on the waiting room wall that read in part, " ...due to the variable response to treatment, we cannot guarantee the results of your treatment. We do guarantee the service you will receive at our clinics. Should our service delivery be less than optimal, please let us know and we will do what is takes to satisfy you, up to and including, crediting your account. " Many patients commented positively on this. Very few actually sought some form of compensation. Our staff was very aware of customer service and worked very hard at it, so I believe this was a factor. I'm not aware of any regs prohibiting a guarantee of results, but I haven't done an exhaustive search of the topic. It will be interesting to see what others have to say about this. Some say that this is the next evolution in medicine: that is, payment for services will ONLY be forthcoming if the outcomes/goals are met under a specific time frame. Interesting, but scary! This raises some issues regarding clinical efficacy vs. patient satisfaction - or are these saying the same thing? It has been observed and surmised that seasoned therapists tend to achieve the best outcomes from interventions stemming at an interpersonal/relational level between therapist and patient - NOT necessarily the therapists' skill set or their choice of clinical intervention. What do you think? Sorry I don't have specific answers but the question is very thought provoking!
