Question about Part B Fee Schedule/Caps

I'm requesting clarification on the Fee Schedule/Caps:

If a patient is on therapy in December and carries over to January, are you

reimbursed at your old rate, or does the fee schedule take effect January 1,

or is it only for NEW patients starting therapy on or after January 1?

Second, regarding the same patient, do they immediately fall under the cap

limitations January 1, or like above, is it only for NEW patients starting

therapy on or after January 1?
