Finally had my appointment with the lawyer (after waiting for about 8 months) He feels that with the CMT diagnosis that I have a better chance the second time around. He mentioned a form for my primary doctor to sign stating that I cannot do sustained employment. Can anyone explain what is all on this form and is there anywhere I can get a copy to look it over? My lawyer is " supposed " to get me one in the next few day, but knowing him itmay take a lot longer. I just thought if i had it to look over a head of time I could plan my strategy!! Also, if my doctor refuses to sign the paper then he want me to go for a " functional capacity evaluation " . Any thoughts on that??? Thanks again!
