I remember one visit in particular. Mother was struggling with supply, baby was slow gaining. I visited to find she had a very active toddler who was havaing difficulty adjusting to a new baby in the house. Mom had no one home with her to help with the toddler, and had a very difficult time sitting down to breastfeed. No wonder she had supply issues and no wonder that baby was slow to gain! If she'd come to my office, she probably would have hired a sitter and I would never have seen this juggling act. Like some others, when I do home visits, I begin charging when I leave my home. I started doing that when I had requests to travel 45-60 minutes away from home, then spent 60 minutes with the mom, and drove home again. I realized I was spending 3 hours, getting paid for 1, and that was not OK by me. And, as someone else pointed out, when you're spending 2 hours in the car, that just took a chunk of time out of your day.
